PE has been defined as persistent ejaculation with minimal stimulation, which is not a result of withdrawal of opiates, and that results in marked distress. DSM IV 1994. This definition assumes regular sexual activity with the same person for around six months. Some definitions include a measure of time to establish the severity of PE. For example, a recent consensus agreement on the definition of lifelong PE includes a time criteria of one minute or less; inability to delay ejaculation on all or most vaginal penetrations; and negative consequences (eg distress, bother, avoidance of intimacy). McMahon CG et al, 2008. The key feature in any definition is the distress that is caused by PE rather than the absolute time between penetration and ejaculation. There is no literature commenting on other forms of sexual activity (oral/anal) or men who have sex with men. The prevalence of PE, based on epidemiological data, is estimated to be 22.7 percent,which is higher than the estimated prevalence of erectile dysfunction. Porst H et al, 2007
