Stress related hematemesis is one of the common complications in critically ill patients admitted to the intensive care unit, the scoring systems that have been developed as scales for evaluation of ICUs function are helpful tools assisting basic decision making and further planning for progression and PRISM-III is one of the best-known scoring systems for assessment of PICU performance ,this study done on all children admitted to the PICU of New Children University Hospital (Abo El riesh , El Mounira ), over a period of 6 months from June 2012 to November 2012 , and 135 patients were included in this study aiming to set a cut off point of PRISM III score in which we can predict stress hematemesis before its occurrence ,and we found that 60% of cases with PRISM III score more than or equal 7 had stress GI bleeding while only 8.5% of cases with PRISM III score less than 7 had stress GI bleeding, also there was a statistically significant correlation between stress GI bleeding and mortality as 82% of cases who had stress related GI bleeding died.
