Tetralogy of fallot (TOF) is the most common congenital cyanotic heart disease. The most common post-operative complication is pulmonary regurgitation PR that may lead to RV dilation and RV dysfunction. We aimed to use tissue doppler echocardiography to assess cardiac systolic and diastolic function as well as ventricular dyssynchrony. Descriptive cross sectional study was conducted on 25 post-operative pediatric patients with successful total surgical repaired TOF. Fifteen sex and age matched healthy children were included as control. Tissue Doppler echocardiography was done to evaluate cardiac functions and ventricular dyssynchrony in addition to clinical evaluation, chest X-ray and 12 leads ECG. RBBB was present in (100%) of cases, prolonged QRS in (92%) and prolonged QTc in 80%
