To study the impact of metabolic derangements on the occurrence of neurological complications following living donor liver transplantation. 90 adult patients of both sexes with end stage liver disease undergoing living donor liver transplantation in kasr al ainy hospital were studied and investigated for the pre - operative period and followed up during post-operative hospital admission and the next 3 months post transplantation. The studied patients were subjected to review of their history before transplantation including the etiology of their liver disease; pre and post transplant laboratory data were documented including serum electrolytes as well as liver and renal functions. Moreover, type of immunosupression used for each patient and levels of those associated with neurotoxicity such as tacrolimus and cyclosporine were studied. Serum electrolyte disturbances had no direct influence on the occurrence of neurological complications while pre transplant hepatic decompensation was the only significant factor affecting the outcome
