Background: Reconstruction of the head and neck following tumor resection poses a major challenge to the surgeon. The search for an ideal flap that can provide superior functional and aesthetic outcomes at both the donor and recipient sites remains an important point of research. Objective: The objective of our study was to assess the role of the thoracodorsal artery perforator (TDAP) flap in head and neck reconstruction and introduce the pedicled TDAP flap. We focused on the uses and advantages of the TDAP with an emphasis on the outcomes at different sites and limitations of using the pedicled flap. Conclusion: The TDAP flap whether pedicled or free plays a role in reconstruction of soft tissue and skin defects of the head and neck. The pedicled TDAP is a promising new technique in head and neck reconstruction that can be used for almost any defect lying below the hard palate, while the free TDAP is a valuable substitute for upper face and scalp defects.
