Introduction: Post-mastectomy radiotherapy (PMRT) for stage II breast cancer is still an area of debate due to criticized existing evidence. Patients and methods: This is an interim analysis of a randomized prospective trial conducted in the N.C.I, in the period from year 2004-2010 involving 99 patients, where patients with stage II breast cancer, who underwent mastectomy were randomized between PMRT & observation. Results: After median follow-up of 49 months, there was no significant difference between both arms regarding OS (p=0.743), MFS (p=0.931) and RFS (p=0.886). Conclusion: Post-mastectomy (PMRT) role in stage II breast cancer needs to be tested in larger, prospective, randomized, more balanced trials
