Contrast enhanced digital mammography CEDM is increasingly used as a diagnostic modality in breast imaging in selected patients. CEDM depict malignant findings by showing pathologic vascularization of the carcinoma. The advantage of CEDM of the breast is its high sensitivity for invasive cancers. Limitations of CEDM include low sensitivity in the detection of IDCS. The described advantages and disadvantages favor the use of breast CEDM in women with high like hood of occult invasive cancers. In our and previous studies, CEDM has been helpful in the differentiation between scar and carcinoma after open biopsy or a tumor recurrence after lumpectomy, it has also been valuable with occult primary breast cancer.Options for the surgical treatment of breast cancer include breast conservative surgery and mastectomy. Factors that influence the choice of surgical therapy include tumor size, ratio of the tumor size to breast size, tumor location, tumor grade, histological findings and personal preference, psychological and cosmetic considerations may also affect the therapeutic decision
