Background and aim: To provide detailed clinic-pathological features of all gastric (GC) and esophageal cancer (EC) cases presented to the national cancer institute (NCI), Egypt (2008 -2012) and evaluation of different treatment modalities, clinical response and outcomes. Patients and methods: Retrospective cohort study carried out on patients files diagnosed with gastric and esophageal malignancies from 2008 to 2012 at the medical oncology department, National cancer institute, Cairo university. Results: This is a retrospective observational study that included 499 cases of esophageal and gastric malignancies. As regard EC, 220 cases with median age of 56 years old, male predominance, two histological types (63.6% squamous cell carcinoma and 36.4% Adenocarcinoma), About 65% of patients presented with stage III-IV. Liver was the most common site of metastasis. The median overall survival (OS), disease-free survival (DFS) and progression-free survival (PFS) were 8.3, 13and 6 months, respectively. Gastric cancer results showed 279 cases, median age is 56 years old, male predominance, histological types (86% Adenocarcinoma and 14% other pathology GIST, lymphoma and carcinoid tumors) .For gastric adenocarcinoma; 52.3% of patients presented with stage III-IV. The median overall survival (OS), disease-free survival (DFS) and progression-free survival (PFS) were 12, 14 and 4 months, respectively. Gastric lymphoma showed two histological types MALT and DLBCL. As median not reached, mean overall survival was 46.9 months. GIST represents 6.5% of all gastric malignancies. As median not reached, mean overall survival was 49.2 months
