Objective: To compare the level of myeloperoxidase enzyme in samples of GCF from uncontrolled type 2 diabetic patients and non-diabetic patients suffering from chronic periodontitis before and after scaling and root planning and its relation to the glycemic control. Methods: Study was conducted on 60 subjects divided into 3 groups, 20 individuals with clinically healthy periodontium (group I), 20 patients suffering from chronic periodontitis (group II), 20 patients suffering from chronic periodontitis and diabetes mellitus type 2 (DM2) with inadequate metabolic control (group III), the following clinical periodontal parameters were assessed before treatment and three month after treatment for all groups to determine the clinical periodontal status in all groups: PD, CAL, GI, VPI and BOP. In all groups, GCF samples were collected at the initial visit and after 3 months after treatment using paper points size 40. GCF myeloperoxidase enzyme levels as assessed using ELIZA technique. Results: Regarding all clinical parameters and the myeloperoxidase level, greater pre-treatment values were recorded in group II and group III (Chronic periodontitis with and without diabetes) in comparison to control. Values were constantly greater in group III (chronic periodontitis with diabetes), except for VPI levels which were slightly higher in group II (Chronic periodontitis without diabetes). After treatment, levels of all parameters decreased. Group III (chronic periodontitis with diabetes) showed higher post treatment levels regarding the clinical parmeters, whereas Group II (Chronic periodontitis without diabetes) revealed higher myeloperoxidase levels
