Objective : The purpose of the study is to review the literature, compare between clipping result of our study and global clipping results on one hand and between clipping results and coiling globally on the other hand. Methods: this is a retrospective study included 10 patients suffered from SAH due to rupture MCA aneurysm .All patients are evaluated clinically and radiologically using CTA,4 vessel angiography preoperatively and control CT brain postoperatively.Results: In this study 10 patients were included: 8 males (80%) and 2 females (20%), where the male: female ratio was 4:1 .The median age of presentation in this study was 44.2 years. Among the 10 patients evaluated according to the side of the pathology (6 of them were right MCA rupture aneurysm (60%), 4 patients were left MCA rupture aneurysm (40%). 7 patients suffered side weakness. 3 patients of them suffered right side weakness and 4 of them suffered left side weakness, All the patients are managed surgically by clipping and 4 patients needed V-P shunt placement. All the patients complained of sudden severe headache as the first symptom. Conclusion: Surgical management of middle cerebral artery aneurysm is still a good alternative
