Study Design Prospective comparative cohort study. Objectives To analyze the effectiveness and practicality of using autologous bone graft enhanced with platelet-rich plasma (PRP) in instrumented fusion with posterolateral fusion for lytic spondylolisthesis. Methods Surgery was undergone on 40 patients, All patients had lytic spondylolisthesis .patients were divided into two groups. The first group named control group, instrumented posterolateral fusion was done using autologous bone graft alone. The second group named study group, Patients in study group underwent instrumented posterolateral fusion with autologous bone graft soaked with PRP. Results All patients were assessed functionally by using the Oswestry disability index, preoperatively, after surgery by three months, six months and twelve months and radiologically by plain X-rays which were done preoperatively and immediate postoperative and at 1, 3, 6, 12 months postoperative, Computed tomography scans when needed of the fused segment postoperatively at 12 months to assess the fusion mass.The data demonstrated that autologous growth factors concentrated during the preparation of PRP, didnt lead to enhanced bone fusion having the same functional outcome and fusion rate if not used with no significant difference in P value. Conclusion PRP used with autologous bone substitute have no effect regarding the functional outcome and the rate of fusion
