Background: Unplanned hospitalization is a marker of poor prognosis and a major driver of costs in patients with cirrhosis. Identification of individuals at high risk of decompensation and hospitalization can therefore have important patient and health system benefits. There are no validated screening tools which accurately determine risk for unplanned hospital admissions in outpatients presenting with liver cirrhosis. Traditional prognostic tools, such as MELD score and Child Pugh, were neither designed nor intended for predicting individual patient's total burden of co-morbidity and their overall "functional reserve". Aim of the study: The objective of our study was to evaluate the bedside frailty screening tool (Short Physical Performance Battery "SPPB") in prediction of unplanned hospitalization or death in outpatients with liver cirrhosis in comparison to the other conventional numerical scores. Methods: Prospective comparative study,fifty eight patients with confirmed liver cirrhosis were recruited from Kasr Al-Ainy, Cairo- University Hospital. Clinical assessment, routine labs, Frailty index using SPPB, Child score and MELD score were calculated on admission, with follow up after 3 months regarding morbidity and mortality
