Background: Azoospermia may be obstructive or non-obstructive with different histopathological causes. Testicular sperm extraction has been useful diagnostic and therapeutic tool with Sperm retrieval rate (SRR) varying according to the pathological finding. Sonoelastography (SE) is an exciting radiologic method that can assess the mechanical properties of tissues and can be used in imaging the elasticity of biological tissues.This radiologic technique can measure relative elasticity of different tissues in a selected region of interest by depending on fast cross correlation technique and a combined autocorrelation method. Real-time Elastography (RTE) can be used for structural analysis of testicular tissue to detect pathological tissue alterations. The aim of this study is to evaluate the efficacy of Shear wave elastography (SWE) in predicting the sperm retrieval rate (SRR) in non-obstructive azoospermia (NOA) patients undergoing testicular sperm extraction (TESE) operation. Patients and methods: This cross sectional study included 50 non obstructive azoospermia male patients aged from 21 to 40 who did two successive semen analysis at least showing no sperms in normal volume ejaculate with normal or elevated gonadotrophic hormones and will undergo Testicular Sperm Extraction operation at Andrology outpatient department in Kasr Al Ainy hospital Cairo University. After history taking, thorough physical examination, and laboratory investigations, all participants were subjected to SWE imaging on both testes by expert assistant professor of radiology in Radiology department in Kasr Alainy hospital. The measurements were recorded in terms of kPa using the SWE mode. They were done on each testis in the longest longitudinal plane. SWE images were viewed using dual mode: elasticity mode (kPa), and propagation (arrival time contour) mode then patients underwent TESE in Kasr Al-Ainy Hospital
