Objective: to assess the efficacy of high power Diode laser and Photodynamic therapy (PDT) on Enterococcus "E" faecalis or Staphylococcus "Staph" aureus biofilms in a novel dentin infection model using confocal laser scanning microscope. Material and Methods: one hundred and twenty extracted human premolars with single canals were instrumented by protaper next up to X4. Teeth were randomly divided into two equal experimental groups according to the type of Biofilms used for infection of the teeth; group 1 (teeth infected with E. faecalis).Group2 (teeth infected with Staph aureus). Biofilms in each group were generated based on a previously established protocol. Each group was further divided into five equal subgroups according to the treatment techniques into; subgroup A: 2% chlorohexidine, subgroup B PDT (650 nm diode laser and methylene blue), subgroup C: 2% chlorhexidine then PDT, subgroup D: 810 nm diode laser with 1 watt for 2 minutes, subgroup E: 2% CHX then 1watt diode laser for two minutes' . Colony forming units and confocal laser scanning microscopy "CLSM" were used to assess bacterial reduction. Data were analyzed by appropriate statistical analyses with P = 0.05. Results: Regarding the type of bacterial biofilm infection, two groups had a significantly high percentage of bacteria reduction (P < 0.05). However, a combination of CHX and 1-watt diode laser for 2 minutes was significantly the highest and CHX alone was the least (P < = 0.001). Conclusion: Under the condition of the present study ; results reinforced that high power diode laser or PDT is a useful adjunct, 810 nm diode lasers or PDT was more effective in eradication of E. faecalis or Staph. aureus biofilms from the root canals than chlorhexidine alone
