Background: The skill of dealing with the different fluids thus offering the hemodynamic aid is crucial while treating critically ill.Transfusion of blood products, especially of packed red blood cells (PRBCs), is an important aspect of care in critically ill children. Aim and objectives: The aim of the study was to assess the common indications of blood transfusion in our PICUs, observing how can blood transfusion affect the outcome of critically ill patients(SOFA score), and to study common complications associated with blood transfusion. Subjects and methods: This was a cross sectional study which was carried out at Pediatric ICUs, Cairo University children Hospital, in the study; Patients needed blood transfusion among all children admitted in PICU were 274 (37%) from total number admitted in ICU which was 739 children. The study was carried out through a period of 6 months. Results: results of the study revealed that Indication for RBCs transfusion was distributed mainly 107(39.1%) anemia for age and 101(36.9%) not anemic but hemodynamically unstable then 66(24.1%) not anemic but hemodynamically stable, while as regards inotropic support score, and total SOFA score Means ±SD were 2.12±1.42, and 7.94±3.61 before blood transfusion, and then became 2.08±1.44, and 7.78±3.65 after blood transfusion respectively
