Rationale: Anemia is a major complication of chronic kidney disease associated with poor quality of life with increased morbidity and mortality. Additionally, it accelerates the rate of progression of chronic kidney disease. Erythropoietin stimulating agents are used to treat anaemia associated with chronic kidney disease; however some patients dont achieve the desired haemoglobin concentration despite higher doses of erythropoietin stimulating agent.Several studies increase awareness of ESA hypo responsiveness, correlate hypo responsiveness to the presence of anti-EPO receptor antibodies and correlate their presence to the severity of anaemia Purpose: This study aims to evaluate the presence of anti EPO receptor antibodies on regular haemodialysis patients on ESA and assess the relationship between their presence and ESA hypo responsiveness Methodology : The present study included 77 patients on regular hemodialysis receiving Erythropoietin stimulating therapy. Patients were recruited from dialysis unit at KasrEl Einy hospital over the period from December 2019 to October 2020.The patients were investigated for CBC, renal function tests, iron profile, PTH and anti EPO receptor antibodies. Results:The study shows that 30% of included cases have anti-ESAR ab, we found no statistically significant relation with demo graphic, clinical and laboratory data except for parathyroid hormone, additionally we found no relation between antibodies and degree of Anaemiaor erythropoietin resistive index
