Background: Hyper-Immunoglobulin E syndrome (HIES) is a primary immune deficiency disorder, which is characterized by eczema, recurrent infections and elevated serum IgE. Several case reports described coronary abnormalities in STAT3-HIES. Aim of the study: this study was conducted to detect coronary abnormalities associated with DOCK8 defect-HIES patients. Methods: This cross-sectional study included fifteen pediatric patients diagnosed with DOCK8 defect-HIES, who were followed up at the Immunology Outpatient Clinic, Cairo University Childrens Hospitals. All patients were subjected to thorough history taking, clinical examination, and immunological profile. Also, cardiac troponin I level, 12-lead electrocardiography, echocardiography, and multislice computed tomography (MSCT) angiography, were performed. Data were collected and subjected for statistical analysis Results: The mean age was 6.80±4.55 years and the male: female ratio was 2:1. Statistical analysis showed that 10 (66%) patients had low CD4%, 8 (53%) patients had low CD3% and 3 (20%) patients had low CD8%. Four (26%) patients had elevated troponin I levels. All echocardiographic parameters (M and 2D modes as well as tissue Doppler) were normal. Both echocardiography and MSCT angiography revealed normal sizes of the right coronary, left coronary, left anterior descending, and circumflex coronary arteries
