Sinonasal malignancies are a group of rare malignancies that remain challenging to treat. Much progress has been made in both treatment and our understanding of the disease process. What is clear from the data is that resection to negative margins is the most important variable predictive of survival that surgeons may in!luence. The endoscopic approach is at least as good as the open approach in obtaining negative margins.The endoscopic approach does carry less morbidity and a shorter hospital stay. The gold standard for surgery of sinonasal malignancies is therefore to select the approach that is most capable of obtaining negative margins while minimizing patient morbidity. It is vital in future studies to consider the great variation in the pathology.of sinonasal tumors. Di"erent pathological types should be investigated as unique diseases with speci!ic criteria. It is also of paramount importance to emphasize on the importance of close follow up, especially in the !irst year of resection, to monitor any recurrence. Further studies should consider collecting data from centers throughout the country (nationalbased studies) to overcome the obstacle of the rarity of sinonasal tumors
