Aim: Open reduction and internal plate fixation of the fibula is the gold standard treatment for ankle fractures.The aim of this study was to perform a prospective randomized study to compare bone union, complications and functional results of two types of internal fixation of the fibula (plating and intramedullary fixation with a screw(. Materials and methods: This prospective case series study involved 52 patients with inclusion criteria: Lateral malleolus fracture according to Weber classification, Weber A and Weber B (isolated or associated with medial malleolar fracture) presenting to the emergency department of Cairo University Hospitals between the periods of November 2020 to June 2021. All patients were followed up for a period of 12 weeks at least, with an average follow up time of 16 weeks.The longest follow up was 7 months. Results: There was no significant difference in the rate of union (P = 0.5605) and functional outcome scores (AOFAS and OMS) between the two types of fixation. Plate fixation group showed adequate reduction than intramedullary group. There were significantly fewer complications with intramedullary fixation than with plate fixation
