Background:Mechanical neck pain commonly arises insidiously and is generally multifactorial in origin, including one or more of the following: poor posture, anxiety, depression, neck strain Purpose: To compare the efficacy of instrument-assisted soft tissue mobilization (IASTM) versus myofascial release on patients with mechanical neck pain. Methods: Forty-five ( 20 male and 25 female) patients aged 25 to 40 years were randomly assigned into 3 equal groups. Each group consistsed of 15 patients. Group (A) received Garston technique plus a selected physical therapy treatment. Group (B) received myofascial release plus a selected physical therapy treatment. Group (C) received the selected physical therapy treatment only. They were evaluated pre and post-treatment for neck pain severity by the visual analogue scale, functional ability using neck disability index, range of motion using an inclinometer, and tenderness by pressure algometer. Results: There was a significant difference post-treatment between instrument-assisted soft tissue mobilization and myofascial release. Instrument-assisted soft tissue mobilization and myofascial release were more effective than the control group in relieving pain,improve functional disability, and increase range of motion, with greater improvement in instrument-assisted soft tissue mobilization group compared with myofascial release group
