Purpose: This study was conducted to compare between the effects of using threshold inspiratory muscles trainer (IMT) devices versus adjustment of the mechanical ventilator (MV) trigger sensitivity on weaning patient from mechanical ventilation. Methods Ninety patients from both sexes, with acute respiratory failure, intubated and mechanically ventilated. Their ages ranged from 50 to 70 years. They were selected from Intensive Care Unit (ICU), Department of Chest Diseases, Cairo University Hospitals. They were randomly assigned into three equal groups. Group (A): included thirty patients who were trained by threshold IMT device plus routine physical therapy. Group (B): included thirty patients who were trained by adjusting (MV) trigger sensitivity plus routine physical therapy. Group (C): included thirty patients who were received only routine physical therapy. (Negative inspiratory force NIF, arterial blood gases, Horowitz index (P/F ration), respiratory rate RR, tidal volume TV, and rapid shallow breathing index RSBI) were measured before the study and at the end of the study (just before weaning for successfully weaned patients, or on the 10th day of intervention for failed weaning patients). Results: Comparison between group (A) and (B) showed a significant improvement in (NIF and RSBI) with preference to group (A), other measurements showed no significant difference. Comparison between group (A) and (C) showed a significant improvement in all measurements except (PH and PaCO2) with preference to group (A). Comparison between group (B) and (C) showed a significant improvement in all measurements except (TV, PH and PaCO2) with preference to group (B).
