The Goal of the Egyptian National Programme for Elimination of Filariasis is to eliminate lymphatic filariasis as a public health problem , by reducing Wuchereria bancrofti microfilariae (MF) in the blood to levels that cannot sustain transmission by Culex pipiens (CxPipiens) The programme is based on mass drug administration (MDA) of annual single - doses of diethylcarbamazine with albendazole (DEC / Alb) to endemic human populations for 5 years , in order to : 1) determine whether patients with low MF counts (1 - 74 MF / ml blood) could bea reservoir of infection after 2 cycles of MDA2) to evaluate the impact of 2 treatment cycles (MDA - 1 and MDA - 2) on Cxpipiens infection in an endemic village (Kafr Tarina) with pretreatment MF prevalence of 9In hundered persentLaboratory reared mosquitoes were fed on 30 volunteers at 3 - month intervals after 2 rounds of MDARates and intensities of MF uptake and infectivity were assessed by dissectionOne year after MDA - 1 , 6 subjects transmitted MF to mosquitoes , but up to 9 months after MDA - 2 transmission failedSix months after MDA - 2 90In hundered persent of treated subjects have cleared MF in 50 l blood smears and they failed to transmit MF , or transmitted MF that did not develop to L3It is concluded that the transmission cycle is seriously weakened after MDA - 2In Kafr Tarina , nine months after MDA - 1 , 609 indoor - resting Cxpipiens from 80 houses were tested for Wbancrofti infection by PCR , whereas after MDA - 2 , 757 females from 61 houses were testedThe proportion of houses containing infected mosquitoes was significantly lower (148In hundered persent) after MDA - 2 , compared to 557In hundered persent recorded after MDA - 1fter MDA - 1 , PoolScreen point estimate of mosquito infection was fairly high (93In hundered persent) , but was reduced to 13In hundered persent after MDA - 2 (8602In hundered persent reduction) After MDA - 2 , the proportion of houses harboring infected mosquitoes was significantly reduced for both the smear - positive (from 50 to 200In hundered persent) and smear - negative households (from 592 to 122In hundered persent) These findings disclosed that although two rounds of MDA with DEC / Alb may not be sufficient to interrupt transmission by Cxpipiens in a village with pretreatment high prevalence of filarial infection , the transmission cycle of the filarial parasite by mosquitoes is seriously impairedIf selected volunteers are representative of microfilaremic subjects in endemic localities and if efforts to achieve high MDA coverage rates can be sustained for the planned 5 years , the present data suggest that the Egyptian programme would more likely lead to the elimination of filariasis as a public health problem
