| Summary, etc. | Type 2 diabetes mellitus is a major risk factor for all types of heart failure (HF) and causes an increase in mortality and morbidity in patients with HF. First phase ejection fraction may be useful in identifying early systolic dysfunction in patient with type 2 diabetes mellitus.<br/><br/>Aim of study: The study aimed to detect Prevalence of Impaired First phase ejection fraction in patients with type 2 Diabetes mellitus.<br/><br/>Patients and Methods: This observational cross-sectional study was performed on patients with Diabetes type 2 who are above 40years old at Cairo university hospitals (Kasr Alainy School of medicine) over the period from September 2020 to June 2022.Every patient underwent conventional transthoracic echocardiography including assessment of first phase ejection fraction .<br/><br/>Results: There was a relation between the incidence of complications and EF1 as patients with complications had significantly lower EF1 when compared to those with no complications (P= 0.036).There was a significant negative correlation between EF1 and duration of DM (r= -0.343, P= 0.006) as the longer the duration of DM was, the lower EF1 was detected .Out of 70 type II diabetic patients, only 5 (7%) had impaired EF1 function (with less than 25% EF1).There was a Relation between HbA1C and EF1 which was statistically significant as patients with uncontrolled DM (>7.5% HbA1C) had significantly lower EF1 as compared to those with controlled DM (<7.5% HbA1C) with P value= 0.04. There was no significant relation between Oral Hypoglycemic Drugs and EF1 (P= 0. 487) and also there was no significant relation between insulin medication and EF1 in the studied patients (P= 0.63) . In this study, the Medial E/E` ratio ranged From 4.4 to 15.4 and the lateral E/E` ratio ranged From 3.2 to 12.8.There was a significant negative correlation between EF1 and mean E/E` (r= -0.279, P= 0.024) as the higher E/E` the lower EF1 results were.<br/><br/>Conclusion: The prevalence of impaired first phase ejection fraction is not high among T2D patients. The lower EF1 results were, more diastolic dysfunction was observed in the studied patients. Uncontrolled, complicated and long duration of diabetes had impaired lower EF1when compared to their counterparts. <br/> |

| Summary, etc. | يتزايد معدل الإصابة بمرض السكري (DM) في جميع أنحاء العالم ويفترض بسرعة أبعاد وبائية. على مدى العقود الثلاثة الماضية ، اقترح عدد من الدراسات الوبائية والسريرية وتشريح الجثة وجود مرض القلب السكري ككيان سريري متميز. يشار إلى فشل القلب الانبساطي (HF) أيضًا باسم HF ، مع الحفاظ على وظيفة انقباض البطين الأيسر. أفادت العديد من الدراسات أن معدل حدوث قصور القلب لدى مرضى السكري مرتفع حتى في غياب ارتفاع ضغط الدم ومرض الشريان التاجي. أبلغت الدراسات عن ارتفاع معدل انتشار الخلل الانبساطي قبل الإكلينيكي بين الأشخاص المصابين بمرض السكري. تشير الأدلة إلى أن تلف عضلة القلب في مرضى السكري يؤثر على الوظيفة الانبساطية قبل الوظيفة الانقباضية.<br/>قد يصاب مرضى السكري من النوع 2 بأمراض القلب والأوعية الدموية الكامنة دون أن يختبروا أو يتعرفوا على العلامات والأعراض المنبهة حتى فوات الأوان. لذلك يصعب دراسة المظاهر تحت الإكلينيكية والإبلاغ عنها ، مما يجعل المقياس الحقيقي للمشكلة سؤالًا مهمًا في علم الأمراض المرتبط والمفهوم جيدًا. <br/>هذه الدراسة تهدف الى معرفة مدى اختلال المرحلة الاولى من الوظيفة الانقباضية للبطين الايسر فى مرضى السكرى من النوع التانى .<br/>تضمن هذه الدراسة حوالى 70 مريض من مرضى السكرى من النوع التانى 45 منهم كانوا من الذكور ومتوسط اعمارهم كانت 52 عاما .تم فحص قلب كل مريض منهم باستخدام الموجات الصوتية بما فيها تقييم المرحلة الأولى للوظيفة الانقباضية للبطين الأيسر .<br/>قد وجد أن خمسة فقط من المرضى يعانون من اختلال المرحلة الاولى للوظيفة الانقباضية للبطين الايسر.<br/> |
