| Summary, etc. | Background: Mastectomy after breast cancer is usually accompanied by shoulder dysfunction which interferes with the daily life activities of the patients and causes physical and psychological impairments. Objectives: the study was conducted to evaluate the impact of scapular mobilization and strengthening exercises on shoulder function post mastectomy. Design: A single blinded randomized controlled trial. Setting: Outpatient clinic. Participants: Forty female patients, their ages ranged from 40 to 55 years, suffering from postmastectomy shoulder dysfunction, were allocated randomly into two equal groups: the study group (group A), managed by scapular mobilization and strengthening exercises besides their traditional physical therapy program (shoulder joint mobilization, posterior capsule stretching, ROM exercises) or the control group (group B), managed by the traditional physical therapy program only. Intervention: Trial was applied for (three sessions/ week) and lasted for four weeks Outcome measures: Shoulder pain and impairment were measured by Shoulder pain and disability index (SPADI), shoulder range of motion and scapular upward rotation were measured by bubble inclinometer pre and post four-weeks of treatment. Results: after 4 weeks of treatment, percentage of change in shoulder pain and disability index (SPADI), upward rotation of scapula, shoulder flexion, abduction and external rotation was 34, 103.55, 45.71, 63.19 and 31.41% respectively in the study group and14.77, 58.42, 22.25, 30.30 and 23.40% in the control group, respectively. In all measures, there was a statistically significant difference in favour of the study group (p< 0.001). Conclusion: It could be concluded that adding scapular mobilization and strengthening exercises are more effective in improving shoulder function regarding pain and ROM post mastectomy than using the conventional physical therapy program only. |

| Summary, etc. | الهدف من البحث هو تقييم التأثير العلاجي لتحريك لوح الكتف و تمارين تقوية عضلات لوح الكتف علي وظيفة الكتف بعد استئصال الثدى.<br/>طريقة البحث:<br/>أجريت هذه الدراسة علي أربعين مريضة تعاني من اختلال وظيفي بالكتف بعد اسئصال الثدي و قد مضي علي العملية من ثلاث الي ستة اشهر. تراوحت أعمارهن بين الأربعين و الخمس و خمسون عاما وقد تم تقسيمهن الى مجموعتين متساويتين فى العدد حيث تضم المجموعة الاولى (مجموعة الدراسة) عشرين مريضة تعاني من اختلال وظيفي في الكتف بعد استئصال الثدي و تم اجراء تمارين لتقوية عضلات لوح الكتف و تحريك للوح الكتف بالاضافة الي البرنامج التقليدي الذي يضم تحريك لمفصل الكتف و اطالة للكبسول الخلفي في الكتف و تمارين حركة المفصل بمعدل ثلاث جلسات اسبوعية لمدة اربعة اسابيع. بينما تضم المجموعة الثانية (المجموعة الضابطة) عشرين مريضة تعاني من اختلال وظيفي في الكتف بعد استئصال الثدي تم اخضاعهن الي البرنامج التقليدي فقط الذي يضم تحريك لمفصل الكتف و اطالة للكبسول الخلفي في الكتف و تمارين حركة المفصل بمعدل ثلاث جلسات اسبوعية لمدة اربعة اسابيع. تم خضوع جميع المشاركين لتقييم قياس آلام الكتف وضعفه عن طريق مؤشر آلام الكتف و الإعاقة (SPADI) ، والدوران التصاعدي للكتف ونطاق حركة الكتف باستخدام جهاز قياس الميل قبل وبعد أربعة أسابيع من العلاج.<br/>نتائج البحث:<br/>وجد فارق ذو دلالة إحصائية في متوسط القيم التي تخص مؤشر الام الكتف و الاعاقة عند المقارنة بين قبل و بعد العلاج في مجموعتي الدراسة و المجموعة الضابطة حيث كان الفارق اكبر في مجموعة الدراسة دلالة علي انخفاض مستوي الالم و العجز بعد اجراء تحريك للوح الكتف و تقوية عضلاته.<br/>وجد فارق ذو دلالة إحصائية في متوسط القيم التي تخص حركة مفصل الكتف عند المقارنة بين قبل و بعد العلاج في مجموعتي الدراسة والمجموعة الضابطة حيث كان الفارق اكبر في مجموعة الدراسة.<br/>وجد فارق ذو دلالة إحصائية في متوسط القيم التي تخص حركة الدوران التصاعدي للكتف عند المقارنة بين قبل و بعد العلاج في مجموعتي الدراسة والمجموعة الضابطة حيث كان الفارق اكبر في مجموعة الدراسة.<br/>الاستنتاجات:<br/>يعد دمج تحريك لوح الكتف و تمارين تقوية عضلاته مفيد في تحسين وظيفة الكتف فيما يخص تقليل الألم و تحسين الحركة بعد استئصال الثدي عند جمعها مع البرنامج التقليدي للعلاج.<br/>انتهت الدراسة الى التوصيات الاتية:<br/> إضافة تحريك لوح الكتف و تمارين تقوية عضلاته الي العلاج التقليدي لمرضي اعتلال وظيفة الكتف بعد استئصال الثدي.<br/>اجراء المزيد من الدراسات المستقبلية علي عدد اكبر من المرضي لايجاد دلالة إحصائية افضل للبيانات.<br/>اجراء المزيد من الدراسات المستقبلية لمدة اكبر من أربعة أسابيع لتقييم الأثر الطويل لتحريك لوح الكتف و تمارين تقوية عضلاته علي مفصل الكتف بعد استئصال الثدي.<br/><br/><br/> |
