Objective: to investigate the effect of immobilization versus early range of motion (ROM) exercises following primary arthroscopic rotator cuff repair secondary to full-thickness tear. Methods: Thirty patients with primary arthroscopic rotator cuff repair secondary to full-thickness tear participated in this study. Patients were subdivided into two groups, fifteen in each group. The first group was group A who received early range of motion exercises; and the second group was group B who received delayed range of motion exercises. Data obtained from both groups 6 weeks, 3 months and 6 months postoperatively regarding shoulder flexion, abduction, internal rotation ROM, scapular protraction, scapular upward rotation, shoulder pain and disability and retear-rate were statistically analyzed and compared. Results: comparing both groups revealed statistically significant increase in flexion ROM at 6 weeks, 3 months and 6 months (p = 0.001), significant increase in abduction ROM at 6 weeks, 6 months (p = 0.001) and 3months (p = 0.007), significant increase in internal rotation ROM at 6 weeks, 3months (p = 0.001) and 6 months (p = 0.003), significant decrease in scapular protraction at 3 weeks (p = 0.003), 3 months and 6 months (p = 0.001), no significant difference in scapular upward rotation at 3 months and 6 months (p > 0.05), significant decrease in SPADI at 6 weeks (p = 0.001) but no significant difference at 3 months (p = 0.55) and 6 months (p = 0.22), no significant difference in retear-rate (p = 0.22) of group A compared with that of group B. Conclusion: Our results suggest that early passive motion should be allowed, because the shoulder ROM were better at all follow-up periods and functional results were better at 6 weeks follow-up but matched at 3- and 6-months follow-ups in early group compared with late group. قيمت هذه الدراسة تأثير التثبيت مقابل تمارين المدى الحركى المبكر لمفصل الكتف بعد إصلاح الكفة المدورة بالمنظار. شارك في الدراسة ثلاثون مريضاً، تتراوح أعمارهم بين 40 إلى 65 سنة. تم تقسيم المرضى إلى مجموعتين من نفس العدد.<br/>المجموعة (أ) تتكون من 15 مريضا قامت بعمل تمارين الحركة المبكرة و التى بدأت بعد أسبوعين من الجراحة.<br/>المجموعة (ب) المكونة من 15 مريضا والتي تأخرت في عمل تمارين الحركة المبكرة الى الأسبوع السادس بعد الجراحة <br/>تم تحليل ومقارنة البيانات التي تم الحصول عليها من كلا المجموعتين بعد 6 أسابيع و 3 أشهر و 6 أشهر من الجراحة فيما يتعلق بكل اتجاهات حركة مفصل الكتف الى الامام او الجانب او الدوارن الداخلي وكذلك المدي الحركي لعظمة لوح الكتف و ألم الكتف و مؤشر العجز و معدل الفشل في الجراحة و قد تم تحليلهم و مقارنتهم إحصائيا.<br/> وقد أثبتت نتائج الدراسة الحالية وجود زيادة واضحة ذات دلالة إحصائية في المدي الحركي لثني الكتف و والاختطاف والدوران الداخلي للكتف عند 6 اسابيع, 3 أشهر و 6 أشهر ما بعد الجراحة و نقصان واضح في تقديم عظمة لوح الكتف عند 6 اسابيع, 3 أشهر و 6 أشهر ما بعد الجراحة و عدم وجود إختلاف في دوران عظمة لوح الكتف لأعلي عند 3 أشهر و 6 أشهر ما بعد الجراحة و نقصان واضح في معامل الالم ومؤشر العجز عند 6 اسابيع و عدم وجود إختلاف فيه عند 3 أشهر و 6 أشهر و عدم وجود إختلاف في معدل الفشل في الجراحة ما بين المجموعة (أ) و المجموعة (ب).<br/> |
