Background: Bladder cancer is one of the most common diseases among old age persons and radical cystectomy is the gold standard treatment. Regional blocks as a part of multi-modal analgesia are considered main strategies of Enhanced Recovery after Surgery decreasing post-operative complications including post-operative pain and post-operative hospital stay. They also have an upper hand over systemic intravenous opioids decreasing the risk for post-operative delirium specialy in old age patients. Thoracic paravertebral plane block, intra muscular quadratus lumborum plane block and erector spinea plane block are among these regional anethesia techniques for pain management. <br/>Study design: randomized clinical trial. <br/>Methods: one hundred and forty ASA II-III patients (40 in each study group) above 18 years old who were scheduled to go under elective radical cystectomy were included to the study. <br/>Intervention: Patients were randomized into three groups as ESPB, QLPB and PVPB. Each block was performed preoperatively on each side with 30 ml 0.25% bupivacaine to the patients according to their groups. <br/>Measurements: cumulative 24 hours morphine consumption, time to first postoperative analgesic dose and VAS score for pain recorded at 0, 15min, 30min, 6h, 12h, 24 hours postoperatively. <br/>Main results: there was statistically difference between PVPB group and both ESPB and QLPB groups for 24- hour morphine consumption, time to first postoperative analgesic dose and VAS score postoperatively at 15min, 30min, 6h and 12h, while there was no significant difference between ESPB and QLPB groups. <br/> |

تعد أورام المثانة احد اكثر الأمراض انتشارا بين كبار السن والتى يعد استئصال المثانة جذريا هو العلاج الأمثل لها. <br/>يعد التخدير العصبى احد الطرق المثلى لعلاج الألم بعد الجراحة وتقليل الإقامة بالمستشفى متفوقة على المسكنات المحقونة وريداً متجنبةً مشاكل أمراض الشيخوخة خاصة فى المرضى كبار السن. <br/>التخدير العصبى للعضلة الناصبة للفقار والعضلة القطنية المربعة والعضلة الجار فقرية أحد الطرق لعلاج اللآلام بعد اجراء الجراحة. <br/>نقارن فى هذه الرسالة الثلاث انواع للتخدير العصبى السابق ذكرها فى المرضى الخاضعين لاجراء استئصال جذرى للمثانة البولية. <br/>تم اجراء الدراسة على مائة وأربعين مريض تم اختيارهم لمقارنة الثلاث انواع من حيث كمية المورفين المستهلكة خلال 24 ساعة بعد العملية ، وقت طلب اول جرعة مسكن من المورفين بعد العملية و درجة مقياس النظير البصرى للألم عند 15 دقيقة و 30 دقيقة و 6 ساعات و 12 ساعة و 24 ساعة بعد العملية. <br/>أظهرت نتائج الدراسة تفوق التخدير للعضلة الجار فقرية على مجموعتى التخدير العصبى للعضلة الناصبة للفقار والتخدير العصبى للعضلة القطنية المربعة من حيث كمية المورفين المستهلكة خلال 24 ساعة بعد العملية و وقت طلب أول جرعة مسكن من المورفين بعد العملية و درجة مقياس النظير البصرى للألم عند 15 دقيقة و 30 دقيقة و 6 ساعات و 12 ساعة و 24 ساعة مع عدم وجود اختلاف جوهرى بين مجموعتى التخدير العصبى للعضلة الناصبة للفقار و التخدير العصبى للعضلة القطنية المربعة. <br/><br/> |
