| Summary, etc. | Fractures of the tibia are the most common long bone fractures; as it<br/>has poor soft tissue coverage and its medial surface is entirely<br/>subcutaneous.<br/>In an average population, there are about 26 tibial fractures per<br/>100,000 of the population per year. Males are more commonly affected<br/>than females. The average age of a patient sustaining a tibial fracture is 37<br/>years, with the males having an average age of 31 years and the females 54<br/>years.<br/>Anteroposterior and lateral radiographs of the tibia should be all that<br/>is required for diagnosis of tibial fractures. It is mandatory that the knee<br/>and ankle are included in the radiographic series as the fracture line may<br/>extend in either joint<br/>Over the years, different surgical approaches have been proposed for<br/>IMN of the tibia. The infrapatellar approach, originally described by<br/>Küntscher in the 1940s, is still largely used. The nail is inserted in the<br/>proximal tibia through or around the patellar tendon with the knee flexed<br/>to 90 degrees. Since then, techniques of nailing in an extended or<br/>semiextended position have been described: the parapatellar approach,<br/>which permits intraarticular or extraarticular introduction of the nail<br/>laterally or medially to the patellar tendon, and the suprapatellar approach,<br/>which allows for nail insertion passing articularly through the quadriceps<br/>tendon.<br/>In avoiding knee flexion, these semiextended techniques allow<br/>neutralization of displacing forces in the proximal tibia, allowing better<br/>exposure of the limb, thus extending the techniques and tools used for<br/>fracture reduction. In addition, a semiextended approach may reduce one<br/>of the most common complications of tibial nailing: anterior knee pain.<br/>The aim of this study was assessment of the intraoperative and<br/>postoperative effectiveness of intramedullary nailing of the tibial fractures<br/>via lateral parapatellar approach in semi-extended position.<br/>We conducted a single arm clinical trial to analyze the outcomes of<br/>tibial shaft fractures treated with a lateral parapatellar approach in the semi-<br/>extended position for intramedullary nail insertion. A total of 26 patients<br/>were admitted to emergency department of Kasr AlAiny who were<br/>diagnosed with isolated tibial fracture with and required Tibial<br/>intramedullary nail insertion surgery |
