| Summary, etc. | Limb length discrepancy (LLD) associated with hip<br/>arthritis can lead to disability including impaired gait, low back pain,<br/>pelvic obliquity, and the need to use shoe lifts for correction. Total hip<br/>arthroplasty (THA) remains the most commonly performed procedure for<br/>patients with hip osteoarthritis, restoration of LLD is an important goal<br/>of any THA procedure as it affects functional outcome.<br/>Patients and methods: Prospective Clinical case series over 4 years<br/>we enrolled 23 cases in our study. All patients suffering hip arthritis with<br/>proximal femoral migration causing clinical true LLD of 2 cm or more.<br/>Primary total hip arthroplasty and additional soft tissue releases was<br/>performed via posterolateral approach. LLD, range of motion (ROM),<br/>pelvic obliquity and Harris hip score (HHS) compared preoperatively and<br/>postoperatively with follow up protocol for one year.<br/>Results: The mean Harris hip score improved from 41 (preoperatively)<br/>to 88.4 (one year postoperatively) . The mean LLD preoperative was<br/>-3.1±0.9 cm, while the mean LLD postoperative was -0.1±0.8 cm. Before<br/>the operation, 11 patients representing 44% had Pelvic obliquity, while<br/>only one case had Pelvic obliquity after the operation. One case had deep<br/>wound infection treated with prosthetic removal, ended with girdle stone.<br/>No postoperative nerve palsy was reported in all patients regardless of the<br/>magnitude of limb length corrected up to 4.1 cm.<br/>Conclusion: Proximal femoral migration can be adequately and safely<br/>corrected with primary total hip arthroplasty and additional soft-tissue<br/>releases resulting in good radiological and functional outcomes |

| Summary, etc. | تعد خشونة مفصل الفخذ المصحوبة بمهاجرة عظمة الفخذ لأعلى من الحالات التي يصعب علاجها كما انها تتسبب في اعاقة المريض بسبب فرق الطول في الطرفين السفليين. ويعتبر تغيير مفصل الفخذ وأستبداله بمفصل صناعى هو الحل الأمثل لمثل هذه الحالات مع أمكانية تعويض فرق الطول أثناء الجراحة.<br/> وهكذا قمنا بدراسة ثلاثة وعشرون حالة ممن يعانون من خشونة بمفصل الفخذ مصحوبة بمهاجرة عظمة الفخذ لأعلى مع وجود فرق في الطول 2 سم أو أكثر وكانت بيانات الحالات كالتالى: ثلاثة عشر ذكرا و عشرة اناث تتراوح أعمارهم بين 22 و 65 عاما بمتوسط43 عاما. وتمت متابعة جميع الحالات لمدة عام كامل بعد الجراحة الا حالة واحدة لم تكمل الدراسة لحدوث التهاب صديدى بالجرح ما بعد الجراحة مما اضطرنا لرفع المفصل الصناعى وتركيب مباعد مضاد حيوى.<br/> بعد الفحص الاكلينيكى للحالات وتحديد فرق الطول بين الطرفيين السفليين ومدى تيبس مفصل الفخذ والاطلاع على الاشعات وقياس فرق الطول عليها وقياس درجة اعوجاج الحوض تم عمل تقييم كامل لوظيفة مفصل الفخذ بمقياس هريس.<br/> تم أجراء جراحة تركيب مفصل فخذ صناعى كامل لجميع الحالات مع تسليك للأنسجة المحيطة بالمفصل لتحسين حركة المفصل ما بعد الجراحة وتعويض فرق الطول بين الطرفين.<br/> وبعد الجراحة تم أعادة تقييم لفرق الطول وحركة المفص وميل الحوض وعمل تققيم لوظيفة المفصل بعمل مقياس هريس و عمل جلسات علاج طبيعى لاطالة الانسجة وتقوية العضلات حول المفصل.<br/> و جاءت النتائج مرضيه حيت تحسن متوسط فرق الطول من 3.1 سم قبل الجراحة الى 0.1 سم بعد الجراحة وتتراوح الزيادة ف الطول من 2.1 سم الى 4.1 سم دون حدوث مضاعفات على الاعصاب او الاوعية الدموية كما لوحظ تحسن في حركة المفصل وتحسن في وظيفته تمثلت في زيادة متوسط مقياس هريس حيث كان المتوسط قبل الجراحة 41 واصبح بعد الجراحة 88.4 . <br/> وبالنسبة للمضاعفات تم رصد حالة التهاب صديدى بالجرح وتم علاجها برفع المفصل و تركيب مباعد مضاد حيوى وحالة فرق في الطول ما بعد الجراحة 2 سم وتم علاجها بارتداء رافع للحزاء .<br/>وفى النهاية لقد أوضحت نتائج البحث أن تركيب المفصل الصناعى للفخذ مع تسليك للأنسجة حول المفصل قادر على علاج حالات خشونة مفصل الفخذ المصحوبة بمهاجرة عظمة الفخذ لأعلى وتعويض فرق الطول حتي 4 سم دون حدوث مضاعفات أو أصابة للأعصاب.<br/> |
