| Summary, etc. | Cardiac surgeries affect the functional capacity, thoracic<br/>mobility and pulmonary function which taking together could influence the<br/>quality of life.<br/>Purpose: This study was conducted to compare between the effect of acapella<br/>and incentive spirometer on cardiopulmonary fitness after heart valve surgery.<br/>Methods: Sixty patients from both sexes who underwent heart valve surgery<br/>(mitral valve mainly) selected from National Heart Institute, Giza, Egypt.<br/>Their ages were ranged from (30-40) years old. The patients of this study were<br/>randomly assigned into three equal groups in numbers (n=20): Acapella<br/>group (A) received acapella while incentive group (B) received incentive<br/>spirometer. Both groups (A and B) offered in addition post-operative<br/>traditional physiotherapy program. Traditional control group (C) received<br/>post-operative traditional physiotherapy program only. FEV1, FVC, Spo2, 6<br/>min walk distance, VO2max, Dyspnea level, pain intensity were measured<br/>before and after the study, and the length of hospital stay was recorded. The<br/>program of treatment for each patient was applied daily starting from the 2nd<br/>postoperative day up to 7 days later. Results: There was a significant increase<br/>in FVC, FEV1, SpO2 and a significant decrease in VAS of group A compared<br/>with that of group B (p < 0.05) and group C (p < 0.001). There was a<br/>significant increase in FVC, FEV1 and SpO2 and a significant decrease in<br/>VAS of group B compared with that of group C (p<0.05).There was a<br/>significant decrease in modified borg scale and a significant increase in<br/>6MWD and VO2max of group A and B compared with that of group C (p <<br/>0.05) while there was no significant difference between group A and B (p ><br/>0.05).There was a significant decrease in length of hospital stay of group A<br/>compared with that of group C (p = 0.01). while there was no significant<br/>difference between group A and B and between group B and C (p > 0.05).<br/>Conclusion: There was superior effect of Acapella (group A) rather than<br/>incentive spirometer (group B) to increase pulmonary functions (FVC and<br/>FEV1), improve peripheral oxygenation (SPO2), and decrease pain intensity.<br/>Therefore, it was recommended to implement the acapella rather than<br/>incentive spirometer to maximize the attained benefits. This induce faster<br/>recovery and hospital discharge, optimize functional capacity, decrease<br/>morbidity, and mortality incidence of those patients. |

| Summary, etc. | أجريت هذه الرسالة للمقارنة بين تأثير الأكابيلا مقابل الحافز التنفسى على اللياقة القلبية التنفسية بعد جراحة صمام القلب حيث شارك فى هذة الرسالة 60 مريض ممن أجرى لهم جراحة لصمام القلب (خصيصا الصمام الميترالى)، تم اختيار المرضى من معهد القلب القومى بالجيزة، مصر. تم تقسيم الحالات عشوائيا الى ثلاث مجموعات متساوية (أ)، (ب)، (ج). تلقت الحالات فى مجموعة الدراسة (أ) الأكابيلا بينما تلقت الحالات فى مجموعة الدراسة (ب) الحافز التنفسى. تلقت كلتا المجموعتين (أ)، (ب) بالاضافة الى ذللك العلاج الطبيعى الروتينى للصدر مع اتخاذ الاحتياطات اللازمة لعظمة القص. تلقت الحالات فى المجموعة الضابطة (ج) العلاج الطبيعى الروتينى للصدر مع اتخاذ الاحتياطات اللازمة لعظمة القص فقط. تم قياس وظائف الرئة وخاصة القدرة الحيوية القسرية، وحجم الزفير القسري فى ثانية واحدة، نسبة الاكسجين بالدم، مسافة المشى المقطوعة خلال ست دقائق، و نسبة استهلاك الاكسجين، مستوي ضيق التنفس وشدة الألم قبل وبعد الدراسة ، وتم تسجيل مدة الإقامة في المستشفى لكل مجموعة. تم تطبيق برنامج العلاج لكل مريض يوميا بدءا من اليوم الأول الذي تم فيه خروج المريض من الرعاية حتى 7 أيام. فى النتائج بعد العلاج وجد ان هناك زيادة ملحوظة ذات دلالة احصائية فى السعة الحيوية القسرية، وحجم الزفير القسرى، وتشبع الأكسجين المحيطى وانخفاض معنوى فى المقياس التناظرى البصرى للالم للمجموعة (أ) مقارنة بالمجموعة (ب) p<0.05 و المجموعة (ج) p<0.001. هناك زيادة ملحوظة ذات دلالة احصائية فى السعة الحيوية القسرية، وحجم الزفير القسرى، وتشبع الأكسجين المحيطى وانخفاض معنوى فى المقياس التناظرى البصرى للالم للمجموعة (ب) مقارنة بالمجموعة (ج)p<0.05. كان هناك انخفاض معنوي في مقياس بورغ المعدل للتنفس وزيادة معنوية في مسافة المشى المقطوعة خلال ست دقائق والحد الأقصى لاستهلاك الأكسجين للمجموعة (أ) و(ب) مقارنة مع المجموعة(ج)p<0.05 بينما لم يكن هناك فرق معنوي بين المجموعة (أ) و (ب)p>0.05. كان هناك انخفاض معنوي في مدة الإقامة في المستشفى للمجموعة (أ) مقارنة مع المجموعة (ج) p=0.01 بينما لم يكن هناك فرق معنوي بين المجموعة (أ) و (ب) وبين المجموعة (ب) و (ج)p>0.05 .<br/> الأستنتاج: يمكن لأكابيلا تحسين اللياقة القلبية التنفسية ، وزيادة حجم الرئة وتشبع الأكسجين ، وخفض مستوى ضيق التنفس وشدة الألم بعد جراحة القلب المفتوح ، ويمكن أن تقلل من دخول المستشفى وتحسين نوعية الحياة<br/> |
