| Summary, etc. | Background: Although laparoscopic surgery had been performed in clinical practice for over 30 years, there has not been much improvement on instruments. Several articulating laparoscopic instruments have been developed including the robotic system. A new multi-degree of freedom (DOF) articulating laparoscopic device has been developed. <br/>Objective: The aim of this study is to assess that articulated laparoscopic instruments are suitable tools for training novices can escalate their laparoscopic skills to meet the experts’ level with suitable training.<br/>Method: A total of 37 participants 17 experts and 20 novices were recruited in our study (Vessalka-Dry Lab) where they were introduced to the articulated instruments and they were viewed tutorial videos by LIVSMED, additional explanation was done for the novice participants for the straight laparoscopic. The participants performed specific required tasks (grasping/Suturing and knotting) using articulated and straight instruments once before a 30 min training session and once afterwards. The time required to perform the task was recorded. The performance was compared between groups as well as the straight and articulated instruments<br/>Results: Among the study participants 73 % were males with more females in the novice group. The novice group were relatively younger than the expert one and it was statistically non-significant. <br/>The novice group took lesser time after the 30 minutes training in doing tasks of grasping 5.5±1.9 vs 6.5±1.9( p=0.005), knotting 2.0±1 vs 1.3±1.1(p<0.001) and suturing 1.3±0.7 vs 1.5±1(p<0.001) using both straight and articulated instruments.<br/> The novice group was matching the expert group in grasping using the articulated laparoscopic instrument (simple laparoscopic task) 6.9±1.7 vs 6.5±1.9 (p=0.005). <br/>The Artisential laparoscopic grasper was non-inferior to the straight one after training in the novice group 8.2±1.6 vs 7±2.2. <br/>The Artisential needle holder was non-inferior to the straight one in knotting and suturing after training in the novice group 2.3±1.2 vs 2.4±1 and 1.7±1 vs 1.5±0.75. <br/>The Artisential laparoscopic grasper was non-inferior to the straight one after training in the Expert group 3.5±1.4 vs 3.3±0.75.<br/> The Artisential needle holder was non-inferior to the straight one in simple and surgical knotting after training in the expert group 1±0.9 vs 0.9±0.4 and 1.2±1.1 vs 0.9±0.7<br/>Conclusion: Articulated laparoscopic instruments can improve dexterity of the novice surgeons with significantly greater learning effect that was observed with the novice group in comparison to the experts |

| Summary, etc. | لقد سلط ظهور الجراحة الروبوتية الضوء على العديد من نقاط التحديد في العمليات الجراحية بالمنظار التقليدية مثل زيادة البراعة ودرجات الحرية بالإضافة إلى القضاء على تأثير نقطة الارتكاز. أثارت التكلفة العالية جدًا للروبوت الجراحي الاهتمام بتطوير أدوات المعصم في مناظير البطن التقليدي. تم تقديم أداة منظار البطن الجديدة متعددة الدرجات من الحرية مفصلية للاستخدام مرة واحدة تحت اسم Artisential. يستخدم المستجيب النهائي لهذه الأداة مفصلين لتمكين قدرة معصم كاملة بزاوية 360 درجة على غرار Da Vinci.<br/>في هذه الدراسة ، تم إجراء تقييم لأدوات مناظير البطن المفصلية لفحص ما إذا كانت أدوات مناسبة لتدريب المبتدئين ويمكنهم تصعيد مهاراتهم بالمنظار للوصول لمستوى الخبراء مع التدريب المناسب.<br/> تم تجميع مجموعه من 37 مشاركًا و 17 خبيرًا و 20 مبتدئًا في دراستنا (Vessalka-Dry Lab) حيث تم تعريفهم على الأدوات المفصلية وشاهدوا مقاطع فيديو تعليمية بواسطة LIVSMED ، وتم إجراء شرح إضافي للمشاركين المبتدئين من أجل المنظار المستقيم . قام المشاركون بأداء مهام محددة مطلوبة (الإمساك / الخياطة) باستخدام أدوات مفصلية ومستقيمة مرة واحدة قبل جلسة تدريب مدتها 30 دقيقة ومرة بعد ذلك. تم تسجيل الوقت المطلوب لأداء المهمة. تمت مقارنة الأداء بين المجموعات وكذلك الآلات المستقيمة والمفصلية.<br/>استغرقت مجموعة المبتدئين وقتًا أقل بعد 30 دقيقة من التدريب في القيام بمهام الإمساك والعقد والخياطة باستخدام كل من الأدوات المستقيمة والمفصلية ، وكانت المجموعة المبتدئة تتطابق مع مجموعة الخبراء في الإمساك باستخدام أداة المنظار المفصلية (مهمة المنظارالبسيطة). المهام المعقدة كان فريق الخبراء متفوقًا فيما يتعلق باستهلاك الوقت وحركة الدقة وسهولة إكمال المهام.<br/>كان المقبض الحرفي بالمنظار غير أدنى من المقبض المستقيم بعد التدريب في مجموعة المبتدئين. لم يكن حامل الإبرة الحرفي أدنى من حامل الإبرة المستقيم في الغرز والخياطة بعد التدريب في مجموعة المبتدئين. كان المقبض الحرفي بالمنظار غير أدنى من المقبض المستقيم بعد التدريب في مجموعة الخبراء. لم يكن حامل الإبرة الحرفي أدنى من المستقيم في الحياكة البسيطة والجراحية بعد التدريب في مجموعة الخبراء |
