| Summary, etc. | Multiple Sclerosis (MS) is a central nervous system demyelinating<br/>disease which causes different neurological manifestations. Depression and<br/>impairment of functional outcomes are major clinical manifestations in MS patients.<br/>Objective: To assess the efficacy of adding neurofeedback training to cognitive<br/>reframing on depression and functional outcomes in patients with multiple sclerosis.<br/>Subjects and Methods: Thirty four MS patients from both sexes were enrolled in the<br/>study. Their ages ranged from 25 to 35 years. They have mild to moderate depression<br/>according to Beck depression inventory (BDI). Patients were randomly assigned into<br/>two equal groups; study group received neurofeedback training added to cognitive<br/>reframing and conventional physical therapy and control group received the same<br/>cognitive reframing and conventional physical therapy only. Treatment sessions were<br/>conducted three times per week for a month. The duration of each session ranged<br/>from for 60 to 90 minutes. Pre and post treatment evaluation was done for depression<br/>using beck depression inventory and for functional outcomes using 25-feet walk test<br/>for lower limb. Results: There was a significant decrease in mean values BDI post<br/>treatment in the study and control groups compared with that pretreatment (p =<br/>0.001). The improvement of BDI mean scores post treatment was more significant in<br/>the study group compared to controls (p = 0.003). There was a significant decrease in<br/>mean values of 25- feet walk test post treatment in the study and control groups<br/>compared with that pretreatment (p = 0.001). The percent of decrease in 25- feet walk<br/>test in study group was 28.55% and that of control group was 15.35%. There was a<br/>significant difference between both groups in mean values of 25-feet walk test post<br/>treatment in favor of the study group (p = 0.01). Conclusion: Neuro feedback training<br/>significantly improves depression and functional outcome in patients with multiple<br/>sclerosis. Hence, it is recommended to consider neuro feedback training in addition to<br/>cognitive reframing and usual physical therapy program in the rehabilitation of MS<br/>patients. |

| Summary, etc. | التصلب المتعدد هو مرض إزالة الميالين في الجهاز العصبي المركزي مما يؤدي إلي تأثيرات عصبية مختلفة. يعتبر الاكتئاب وضعف النتائج الوظيفية من المظاهر السريرية الرئيسية لدى مرضى التصلب المتعدد. الهدف: تقييم فعالية إضافة تدريب التغذية الراجعة للتعديل العصبي إلي التجديد الإدراكي علي الإكتئاب والنتاج الوظيفي لدى مرضى التصلب المتعدد. الأشخاص والطرق: شارك أربعة وثلاثون مريضا مصابا بالتصلب المتعدد من كلا الجنسين في هذة الدراسة. تراوحت أعمارالمرضي بين 25 و 35 عاما، وكان لديهم اكتئاب خفيف إلى متوسط وفقًا لمقياس بيك للإكتئاب . تم تقسيم المرضى بشكل عشوائي إلى مجموعتين متساويتين ؛ تلقت مجموعة الدراسة تدريبًا على الارتجاع العصبي مضافًا إلى إعادة الصياغة المعرفية مع تلقى العلاج الطبيعي التقليدي وتلقت المجموعة الضابطه إعادة الصياغة المعرفية مع العلاج الطبيعي التقليدي فقط. أجريت جلسات العلاج ثلاث مرات في الأسبوع لمدة شهر. تراوحت مدة كل جلسة من ٦٠ إلى ٩٠ دقيقة. تم إجراء تقييم ما قبل وبعد العلاج للاكتئاب باستخدام مقياس بيك للإكتئاب وتقييم النتاج الوظيفي باستخدام إختبار مشي بطول ٢٥ قدمًا للطرف السفلي. النتائج: كان هناك انخفاضا ذا دلالة إحصائية في متوسط قيم جرد بيك للإكتئاب بعد العلاج في مجموعة الدراسه والمجموعة الضابطه (إحتماليه= ٠,٠٠١) وكان التحسن في متوسط قيم جرد بيك للإكتئاب بعد العلاج أكثر بفارق إحصائي في مجموعة الدراسة مقارنة بالمجموعة الضابطه (إحتماليه =٠,٠٠٣). كان هناك انخفاض ذا دلالة إحصائية في متوسط قيم إختبار المشي ٢٥ قدما بعد العلاج في مجموعات الدراسة والمجموعة الضابطة (إحتماليه=٠,٠٠١) وكانت نسبة الإنخفاض في قيم اختبار المشي لمسافة ٢٥ قدمًا في مجموعة الدراسة ٢٨.٥٥٪ والمجموعة الضابطة ١٥.٣٥٪. كما كان هناك فارق إحصائي بين المجموعتين في القيم المتوسطة لاختبار المشي ٢٥ قدمًا بعد العلاج لصالح مجموعة الدراسة (إحتماليه = ٠.٠١). الخلاصة: التدريب على التغذية الراجعة العصبية يحسن بشكل كبير الإكتئاب والنتاج الوظيفي لدى مرضى التصلب المتعدد. ولذا فمن المستحسن إضافة هذا التدريب إلى إعادة الصياغة المعرفية وبرنامج العلاج الطبيعي المعتاد في إعادة تأهيل مرضى التصلب المتعدد. |
