| Summary, etc. | Purpose: To assess the efficacy of adding Ahmed’s subflap mattress suture to deep sclerectomy surgery and compare it to the conventional deep sclerectomy surgery.<br/>Design: A prospective randomized controlled trial.<br/>Methods: Our study included 40 eyes from 28 patients with open angle glaucoma whose IOP was not controlled despite maximally tolerated medical therapy. Patients were assigned randomly into two groups:<br/>Group 1: underwent deep sclerectomy with Ahmed’s subflap mattress suture.<br/>Group 2: underwent conventional deep sclerectomy.<br/>Patients were followed up closely for 6 months with IOP recorded at 1 week, 1 month, 3 months & 6 months and ultrasound biomicroscopy (UBM) was used to assess the surgical site functionally and anatomically at the first and sixth month.<br/>Results: In our study, we found that adding Ahmed’s subflap mattress suture improved the IOP lowering effect of deep sclerectomy surgery from 43% in group B to 53% in group A at 6-month post-operative follow up visit and that difference was statistically significant (p= 0.027). Also, eyes in group A ended with having statistically significantly lower IOP at 1 week, 1 month and 6 month visits (7.9 ± 1.3,<br/>11.7 ± 2.2 & 13.3 ± 1.9 mmHg respectively) compared to group B (10.1 ± 4.6, 14.1 ±<br/>5.2 & 16.8 ± 4.1 mmHg respectively) (p= 0.025, 0.041 & 0.001 respectively). All the 4 UBM parameters (whole bleb anteroposterior length & height and intrascleral lake length & height) were statistically significantly larger in group A at 1 & 6 months. Strong statistically significant negative correlations were established between IOP and all the UBM parameters apart from intrascleral lake height at the first and sixth month (p < 0.01 in all of them). Weaker correlations were found between IOP and the intrascleral lake height at both the first and sixth month follow up visits (p= 0.037 &<br/>0.033 at first and sixth month respectively). Finally, significant correlations were found between IOP at 6 months and whole bleb anteroposterior length and height at 1 month (p=0.001) which indicates that measuring the whole bleb dimensions by UBM at 1 month can be used a predictive indicator of IOP later at 6 months.<br/>Conclusion: We recommend adding Ahmed’s subflap mattress suture to routine deep sclerectomy surgery as it is an effective simple economical addition that will enhance the IOP lowering effect of deep sclerectomy surgery. Also, assessment of the bleb by UBM is useful in predicting the success of deep sclerectomy surgery |

| Summary, etc. | المياه الزرقاء هي أهم الأسباب الرئيسية للإصابة بالعمى الدائم. واحد من كل عشرة مرضى سوف يصل الى العمى التام بالعينين، لذلك التشخيص المبكر والمتابعة الجيدة للمرض مهمان لإعطاء المريض العلاج المناسب وإبطاء تطور المرض.<br/> تعتبر عملية القضم العميق للصلبه من الجراحات المهمه في علاج المياه الزرقاء مفتوحة الزاويه وقد اثبتت فاعليتها في خفض ضغط العين مع معدل اقل في المضاعفات عن غيرها من الجراحات.<br/> في هذه الرسالة درسنا مدى فاعلية اضافة غرزة أحمد الى جراحة القضم العميق للصلبة لتحسين نتيجة الجراحة في خفض ضغط العين. تم اجراء الدراسه على 40 عين لمرضى المياه الزرقاء مفتوحة الزاوية الذين يعانون من ارتفاع ضغط العين على الرغم من استخدام القطرات المخفضة لضغط العين وقد تم تقسيمهم الى مجموعتين من المرضي: المجموعه الاولى خضعت للقضم العميق للصلبة مع اضافة غرزة أحمد والمجموعه الثانيه خضعت لجراحة القضم العميق للصلبة بالتقنيه التقليدية.<br/> تم متابعة المجموعتين عن كثب لمدة 6 أشهر مع تسجيل ضغط العين بعد أسبوع وبعد شهر وبعد ثلاثة أشهر وبعد ستة أشهر مع عمل تصوير باستخدام الفحص المجهري بالموجات فوق الصوتية على مكان الجراحة بعد شهر وبعد ستة أشهر.<br/> وتبين من الدراسة انخفاض ضغط العين بعد ستة أشهر في المجوعة الأولى بنسبة 53% وفي المجموعة الثانية بنسبة 43% وكان الفارق ذا دلالة احصائية. وأيضا كان قياس ضغط العين أقل في المجموعة الأولى عن المجموعة الثانية بعد أسبوع وبعد شهر وبعد ستة أشهر من الجراحة وكان الفارق أيضا ذو دلالة احصائية.<br/> ووجدنا فارق ذو دلالة احصائية في قياسات طول وارتفاع الحويصلة وأيضا في طول وارتفاع المساحة البينية بداخل الصلبة بين المجموعتين بعد شهر و ستة أشهر من الجراحة وكانت جميع القياسات أكبر في المجموعة الأولى. وأيضا تم ايجاد علاقة ترابط نسبي عكسي ذو دلالة احصائية بين ضغط العين و قياس طول الفقاعة وارتفاعها باستخدام الفحص المجهري بالموجات فوق الصوتية بعد شهر وستة أشهر من الجراحة. وأيضا وجدنا علاقة ترابط نسبي عكسي بين ضغط العين في الشهر السادس و قياسات الحويصلة (الطول والارتفاع) باستخدام الفحص المجهري بالموجات فوق الصوتية مما يعني ان قياس طول وارتفاع الفقاعة باستخدام الفحص المجهري بالموجات فوق الصوتية قد يساعد في التنبؤ بضغط العين لاحقا عند الشهر السادس.<br/> ومن ثم ننصح باضافة غرزة أحمد الى جراحة القضم العميق للصلبة بشكل دائم نظرا لكونها اضافة بسيطة واقتصادية وفعالة في تحسين فاعلية جراحة القضم العميق للصلبة في خفض ضغط العين. |
