Summary, etc. Background: Osteoporosis is a metabolic bone disease characterized by low bone mass, deterioration of bone tissue, and disruption of bone microarchitecture, which compromises bone strength and increases the risk of fracture in women. The average age of menopause is 51 years, and it typically affects women 10–15 years after menopause. Menopause is a normal physiological shift caused mostly by the aging-related decline in ovarian function, which results in a reduction in the production of the major hormone for maintaining bone density—estrogen—which is responsible for ongoing bone remodeling. Exercise provides mechanical stimulation, antigravity loading, and stress on the musculoskeletal system, which stimulates bone formation and helps maintain bone mass.<br/>Aim of Study: To ascertain the relationship between Estrogen and bone mineral density (BMD) in osteoporotic women and the impact of aerobic exercise on these variables. Patients and Methods: The research and control groups were each made up of 52 post-menopausal women with osteoporosis who were 65 to 75 years old. Both groups were on medicine and had diet plans, but the study group also did aerobic exercise for 12 weeks in addition to their prescription and diet plans. Dual-energy x-ray absorbmetry (DEXA) T-score and serum oestrogen hormone level were used to calculate BMD through laboratory research. Results: The comparison of the experimental data highlights the impact of aerobic exercise on the two groups' Estrogen hormone and bone mineral density outcomes. The experimental exercise group's BMD findings in DEXA and Estrogen hormone showed a substantial improvement, according to the data (p <0.05). Additionally, percentage of improvement for study group for T-score was 9.73% and for serum Estrogen was 9.73%, Also, there was a strong positive correlation between BMD and Estrogen hormone (p =0.039). Conclusion: It was concluded that adding aerobic exercise to osteoporotic post-menopause women's rehabilitation program was effective in improving the T-score of DEXA and serum estrogen level in osteoporotic post-menopause women which its decrease in postmenopausal women considered the most important cause for osteoporosis in this age.<br/>
