Discogenic Sciatica is defined as low back pain radiating to<br/>the lower limbs. It is combined with one or more positive neurological signs.<br/>Sciatic pain causes loss of function and reduces quality of life and working<br/>capability. Objective: The aim of the study was to investigate the effect of<br/>lumbar proprioception training on both cortisol and pro inflammatory<br/>cytokines levels in addition to pain and functional ability in patients with<br/>lumbar radiculopathy . Subjects and Methods: Thirty patients with lumbar<br/>radiculopathy from both sexes participated in this study after signing the<br/>institutional consent form. Their age ranged from 30-45 years old. Patients<br/>were randomly assigned into two equal groups [control group (G1) and<br/>study group (G2). Control group (G1) treated with selected physical therapy<br/>program including TENS, Ultrasonic and lumbar stability exercise. Study<br/>group (G2) treated with proprioception training by isokinetic in addition to<br/>the same selected physical therapy program. All participants were assessed<br/>for cortisol and tumor necrosis factor (TNF alpha) levels. Also, the<br/>participants were assessed for pain level by visual analogue scale (VAS) and<br/>the functional performance by modified oswestry low back pain<br/>questionnaire. Treatment was conducted three sessions per week for six<br/>weeks. Pre and post assessment for all variables were done. Results: There<br/>was significant decrease in cytokines level and pain level as well as a<br/>significant improvement in the functional performance in the two groups<br/>with a notable improvement in favor to the study group. Significant<br/>reduction in cortisol levels post-treatment in study group compared to<br/>control group. Significant positive correlation between cortisol level with<br/>IV<br/>pain and functional disability in study group. Significant positive correlation<br/>between inflammatory cytokines with pain and functional disability in study<br/>group (P <0.05). Conclusion: Proprioceptive training with isokinetic<br/>dynamometer has a positive effect on reducing both cortisol and<br/>proinflammatory cytokines levels and it is an effective method to decrease<br/>pain and increase function in patients with lumbar radiculopathy. Reducing<br/>cortisol and cytokines levels is associated with reducing pain intensity and<br/>improves functional ability in LBP patients. |

•لتحديد تأثير التدريب بجهاز الايزوكينتك على مستوى الكورتيزول في المرضى الذين يعانون من اعتلال الجذور القطنية.<br/>•للكشف عن آثار التدريب بجهاز الايزوكينتك على السيتوكينات المنشطة للالتهابات TNF alpha))في المرضى الذين يعانون من اعتلال الجذور القطنية. <br/>•دراسة تاثير التدريب بجهاز الايزوكينتك على الألم في المرضى الذين يعانون من اعتلال الجذور القطنية.<br/>•التعرف على تأثيرالتدريب بجهاز الايزوكينتك على القدرة الوظيفية في المرضى الذين يعانون من اعتلال الجذور القطنية.<br/>طريقه البحث:<br/> ثلاثون مريضا من كلا الجنسين يعانون من اعتلال الجذور القطنية.تراوحت أعمار المرضى بين 30-45 سنة و كانت المدة من بداية الألم من 3 - 12 شهر وتم إجراء اختبار الكورتيزول في الدم واختبار السيتوكينات المنشطة للالتهابات (عامل نخر الورم الفا TNF) لجميع المرضى .تم تقسيم المرضى الذين تم اختيارهم بشكل عشوائي إلى مجموعتين متساويتين المجموعه الضابطه (G1) ومجموعة دراسة (G2) كانت مدة البرنامج العلاجي لجميع المرضى في المجموعتين 3 جلسات أسبوعياً ولمدة 6 أسابيع.عولجت المجموعة الضابطه (G1) عن طريق التحفيز الكهربائي للأعصاب عبر الجلد والموجات فوق الصوتيه وتمارين ثبات العمود الفقري القطني تم علاج مجموعة الدراسه (2G)عن طريق التدريب الحسي القطني بجهاز الايزوكينتك بالإضافة إلى التحفيز الكهربائي للأعصاب عبر الجلد و تمرين ثبات العمود الفقري القطني و الموجات فوق الصوتيه.<br/> |
