Summary, etc. Purpose: Comparison between the SBK microkeratome versus Femtosecond laser in flap creation during LASIK surgery for myopia.<br/>Design: Prospective comparative non randomized interventional clinical study.<br/>Methods: This is a Prospective comparative non randomized interventional clinical study that included 60 eyes of 30 patients in each group (SBK, FS200 laser group). The patients’ age ranged from 18 to 46 years in the FS200 laser group, and from 20 to 44 years in the SBK group. Flaps were created in 60 eyes of 30 patients using the Wave Light FS200 femtosecond laser and in 60 eyes of 30 patients using the Moria SBK microkeratome (Moria SA, Antony, France). AS-OCT was used 1 month postoperatively to evaluate the thickness of 5 points across each flap, which were 0, 1.5 mm to the corneal vertex on the horizontal and vertical meridians. Hinge size and flap diameter were measured using caliper one month after surgery. UCVA and BCVA were measured at 1 day, 1 week, 1 month post-operative in each group. Any related complications were recorded in each group. For side-cut angles, scans were done across the temporal side of the flap, and then images were transferred to Draw editor software, and measurements were recorded and analyzed 1 month after surgery using AS-OCT.<br/>Results: The mean achieved central corneal flap thickness in both eyes in FS200 laser group was 117.4µ ± 4.18, and 84.90µ ± 3.74 in the SBK group, which was statistically significant (p <0.05). The mean achieved Hinge size in both eyes in FS200 laser group was 3.47mm ±0.26, and 4.81mm ±0.63 (p <0.05) in the Moria SBK group. The mean achieved flap diameter in FS200 laser group was 7.95mm±0.43, and 9.15mm±0.21 in SBK group (p <0.05). The mean achieved side cut angle in FS200 laser group 126.93º±9.56, and 37.18º±6.93 in SBK group (p<0.05).<br/>Conclusion: There was a statistically significant difference between the intended and the achieved side cut angles between both study groups. Flap thickness was more predictable in FS200 laser group compared to SBK group. There were reported complications in the SBK group, while Femtosecond laser technology has shown better postoperative flap uniformity with no reported complications
