| Summary, etc. | Background: Rectal intussusception is an infolding of the rectal wall that may occur during the act of defecation. In patients with symptoms of rectal evacuatory dysfunction, rectal intussusception is a common finding at evacuation proctography. It can cause symptoms of obstructed defecation, fecal incontinence, incomplete rectal evacuation and pain. Stapled trans-anal rectal resection using PPH staplers is widely used in the treatment of intussusception with good results.<br/>Purpose: To evaluate the safety, efficacy and feasibility of Stapled Trans-anal Rectal Resection (STARR) procedure performed by a single or double stapler through clinical and functional outcomes for trans-anal stapled surgery.<br/>Methods: Our study is a randomized controlled trial which included 48 patients with rectal intussusception. They were divided into two groups; one group underwent STARR operation using single PPH stapler anteriorly and the other group underwent STARR operation using double PPH staplers.<br/>Results: Both groups showed significant improvement in obstructed defecation syndrome score with a P value <0.0001 in both groups, comparing between the two groups the results were comparable regarding the obstructed defecation syndrome score improvement with a P value = 0.211 at 3 months and = 0.282 at 6 months. The operative time in the single STARR group was 30.8 ± 3.1 while in the double STARR group was 44.6 ± 4. VAS pain score was 2.6 ± 1.5 in the single STARR group and 3 ± 1.5 in the double STARR group. Defecation urgency reported in 2 patients in the single STARR group and 8 patients in the double STARR group. No postoperative bleeding occurred in the single STARR group while one patient reported minor bleeding in double STARR group managed conservatively. Transient incontinence to flatus occurred in 1 patient in the single STARR group and 3 patients in the double STARR group.<br/>Conclusion: STARR procedure using single stapler showed comparable results with double staplers regarding obstructed defecation syndrome score improvement with less operative time, cost and postoperative defecation urgency. |

| Summary, etc. | الانغلاف المستقيمي هو انتفاخ لجدار المستقيم قد يحدث أثناء التبرز. في المرضى الذين يعانون من أعراض ضعف إفراغ المستقيم، يعتبر انغلاف المستقيم نتيجة شائعة في تصوير المستقيم اثناء الإخلاء. يمكن أن يسبب أعراض الإمساك الانسدادى وسلس البراز وإخلاء المستقيم غير الكامل والألم. يستخدم استئصال المستقيم بالتدبيس عبر الشرج باستخدام دباسات على نطاق واسع في علاج الانغلاف بنتائج جيدة. <br/>الغرض: لتقييم سلامة وفعالية وجدوى إجراء استئصال المستقيم عبر الشرج الذي يتم إجراؤه بواسطة دباسة مفردة أو مزدوجة من خلال النتائج السريرية والوظيفية للجراحة بالتدبيس عبر الشرج.<br/>الطريقه: دراستنا عبارة عن تجربة عشوائيه محكومه شملت 48 مريضًا يعانون من انغلاف المستقيم. تم تقسيمهم إلى مجموعتين؛ خضعت إحدى المجموعات لعملية استئصال المستقيم عبر الشرج باستخدام دباسة واحدة من الأمام وخضعت المجموعة الأخرى لعملية استئصال المستقيم عبر الشرج باستخدام دباسات مزدوجة.<br/>النتائج: أظهرت المجموعتان تحسنًا ملحوظًا في نتيجة متلازمة التبرز المسدود، مقارنة بين المجموعتين كانت النتائج قابلة للمقارنة فيما يتعلق بتحسن درجة متلازمة التبرز المسدود، كان وقت الجراحه في مجموعة الدباسه المفردة 30.8 ± 3.1 بينما كان في مجموعة الدباسه المزدوجة 44.6 ± 4. كانت درجة الألم 2.6 ± 1.5 في مجموعة الدباسه الفردية و3 ± 1.5 في مجموعة الدباسه المزدوجة، اثبت وجود اعراض إلحاح التبرز في 2 مرضى في مجموعة الدباسه واحدة و8 مرضى في مجموعة الدباسه المزدوجة، لم يحدث أي نزيف بعد العملية الجراحية في مجموعة الدباسه المفرده بينما أبلغ مريض واحد عن نزيف طفيف في مجموعة الدباسه المزدوجة التي تمت إدارتها بشكل تحفظى. حدث عدم تحكم مؤقت فى الريح في مريض واحد في مجموعة الدباسه المفرده و3 مرضى في مجموعة الدباسه المزدوجة.<br/>الاستنتاج: اظهرت دراستنا ان إجراء استئصال جزء من المستقيم عبر الشرج باستخدام دباسة مفرده له نتائج قابلة للمقارنة مع دباسات مزدوجة فيما يتعلق بتحسين نقاط متلازمة الامساك الانسدادى مع وقت أقل للعملية واقل تكلفة واقل حدوث للإلحاح التبرز بعد العملية الجراحية. |
