Summary, etc. | Background: Aminoglycosides are important components of antipseudomonal chemotherapy. The inactivation of drugs by modifying enzymes is the most common mechanism of aminoglycoside resistance. The aim of this study was to examine the prevalence of aminoglycoside modifying enzyme genes in clinical isolates of Pseudomonas aeruginosa at Cairo University Hospitals (Japanese pediatric hospital) with respect to the site of infection and microbial resistance of the strains.<br/>Methods: A cross-sectional study was carried out on all Pseudomonas aeruginosa isolates collected from May 2017 to July 2018 until 100 resistant isolates which are resistant to one or several aminoglycosides were selected. Antimicrobial susceptibility tests (using the disk diffusion method) were performed. In addition, all isolates were screened for the presence of modifying enzyme genes (aac(3)-I and aac(6)-Ib) by polymerase chain reaction.<br/>Results: The resistance rates, as determined by the disk diffusion method, were as follows: tobramycin 100%, gentamicin 98% and amikacin 77%. PCR results revealed that the presence of aac(6′)-Ib was (17 %) among all P. aeruginosa isolates while for aac (3)-I gene the percentage was (1%).<br/>Conclusion: Coordinated efforts and further research works are needed to control antibiotic resistance to aminoglycosides before being a threatening crisis<br/><br/> |

Summary, etc. | مقدمه: الأمينوجليكوزيدات هي عنصر مهم في العلاج الكيميائي المضاد للسيدوموناس. يعد تعطيل الأدوية عن طريق تعديل الإنزيمات الأكثر شيوعًا لمقاومة الأمينوجليكوزيدات.<br/>الهدف من الدراسة هو فحص انتشار جينات الإنزيم المعدلة للأمينوجليكوزيد في العزلات السريرية من السيدوموناس ايريجينوزا في مستشفيات جامعة القاهرة (مستشفى الأطفال الياباني) فيما يتعلق بموقع العدوى والمقاومة الميكروبية للسلالات.<br/>المنهج: أجريت هذه الدراسة على كل عينات السيدوموناس ايريجينوزا في الفترة من مايو 2017 الى يوليو 2018 حتى تم عزل 100 من العزلات المضاده لواحد او أكثر للأمينوجليكوزيدز <br/>تم إجراء اختبارات الحساسية لمضادات الميكروبات (باستخدام طريقة انتشار القرص). بالإضافة إلى ذلك، تم فحص جميع العزلات لوجود جينات إنزيم معدلة<br/> بواسطة تفاعل البلمرة المتسلسل.(aac(3)-I and aac(6)-Ib)<br/> نتائج البحث: معدلات المقاومة، كما تحددها طريقة انتشار القرص، على النحو التالي:توبراميسسن 100%، جينتاميسين 98% و اميكاسين 77%. أظهرت نتائج تفاعل البلمرة المتسلسل وجود الجين aac(6)-Ib في 17 % من جميع عزلات السيدوموناس ايريجينوزا بينما كانت بالنسبة للجين aac(3)-I في 1 % فقط.<br/>الخلاصة: هناك حاجة إلى جهود منسقة وأعمال بحثية إضافية للسيطرة على مقاومة المضادات الحيوية للأمينوجليكوزيدات قبل أن تصبح أزمة خطيرة.<br/><br/> |
