| Summary, etc. | Our observational study was necessary to provide information about <br/>real-world patient characteristics, patterns of care and adherence to <br/>guidelines. This information can impact selection of systemic treatment, <br/>prognosis and quality of care for MBC.<br/>Luminal, hormone receptor-positive human epidermal growth factor <br/>receptor 2-negative tumors are the most frequent subtype of breast cancer. <br/>Even within this subtype, there is a lot of heterogeneity, which complicates <br/>accurate patient selection for therapeutic decision-making.<br/>Our study revealed overuse of CTH as first-line treatment (44%) for <br/>HR+/HER2- MBC, which is against the current guidelines. Higher response <br/>rates (84%) and faster response associated with CTH may induce clinicians <br/>to prescribe first-line CTH.<br/>Being younger (p 0.007), premenopausal (p 0.114), medically free (p <br/>0.027), presence of visceral metastasis (p <0.001) and presenting with more <br/>than one metastatic site (p 0.005) were the most significant factors <br/>influencing the choice of CTH as first-line treatment. <br/>With the same survival outcomes to HT (Median PFS of 15 months and <br/>OS of 42 months) and being more toxic. This is highlighting the need for <br/>greater promotion of adherence to current guidelines. For patients with or <br/>without visceral disease and proof of endocrine resistance, HT (combined <br/>with targeted drugs) is the preferred regimen. <br/>For patients with progression on prior HT, combination of HT and <br/>targeted drugs is recommended as the preferred option. CTH is appropriate <br/>for those diseases requiring rapid response, such as visceral crisis or rapidly <br/>progressive diseases.<br/> |

| Summary, etc. | إن سرطان الثدي مشكلة صحیة عالمیة، وأكثر أنواع السرطان شیوعا لدى النساء، وھو ثاني أكثر <br/>أسباب الوفاة شیوعا بین النساء في العالم. في مصر وفقا للسجل القومي لتسجیل الاورام الذي نشر في <br/>عام ،۲۰۱٤ كان سرطان الثدي ھو أكثر أنواع السرطان شیوعا بین النساء بنسبة ٪۳۲ ووفقا <br/>لجلوبوكان ۲۰۲۰ كان عدد حالات سرطان الثدي بین النساء بمصر ۲۲ ألف حالة بنسبة ٪۳۲٫٤ . <br/>سرطان الثدي لیس نوع واحد، بل مجموعة من الأنواع البیولوجیة المختلفة طبقا لمستقبلات <br/>الاستروجین والبروجسترون وبروتین ھیر ۲ ولھا سلوك واستجابة مختلفة لأنواع العلاج المختلفة <br/>كالعلاج الھرموني او العلاج الكیمیائي أو العلاج الموجھ. <br/>اثبتت الدراسات الترابط بین اورام الثدي مع عوامل جینیة وغیر جینیة وعومل اخري مثل التقدم <br/>بالعمر والتعرض للإشعاع والانجاب والسمنة ووجود تاریخ مرضي للأورام في العائلة. <br/>الثلثین من حالات سرطان الثدي یكون ذات مستقبلات ھرمونیة موجبة سالبة بروتین ھیر۲ التي غالبا <br/>ما یظھرون استجابة للعلاج الھرموني والعلاج الموجھ ال ذي یحجم الورم ویحسن اعراض السرطان <br/>مع اعراض جانبیة اقل عن العلاج الكیمیائي الذي یتمیز بسرعة الاستجابة ولكن مع اعراض جانبیة <br/>أكثر. <br/>تنصح الارشادات العالمیة الي علاج سرطان الثدي المنتشر ذات المستقبلات الھرمونیة الموجبة، سالبة <br/>بروتین ھیر ۲ بالعلاج الھرموني كخط اول مالم یكن المریض بحاجة لاستجابة سریعة لوجود خلل <br/>بالأعضاء الحیویة او حدوث فشل وشیك. <br/>ولكن بالنظر للممارسة الطبیة الیومیة ومناظرة المرضي فان اختیار خط العلاج الأول یتأثر بعدة <br/>عوامل وتھدف ھذه الدراسة إلى وصف العوامل المؤثرة في اختیار العلاج الكیمیائي أو العلاج <br/>الھرموني كعلاج خط أول لمرضي سرطان الثدي المنتشر ذات المستقبلات الھرمونیة الموجبة، سالبة <br/>بروتین ھیر . |
