| Summary, etc. | Background: Chronic Mechanical low back pain (CMLBP) can limit function and ability in individuals with regard to life and work. Therefore, it is recognized as a critical health problem globally. Objective: this study aimed to investigate the effect of diaphragm strength on chronic mechanical low back pain (CMLBP) patients. Methods: Forty-three patients aged 20 to 40 years old with chronic mechanical low back pain participated in the study. The sample collected from EL Kasr EL-Ainy hospital out clinic Patients were assigned into two groups by simple random sampling pre- test post- test group randomized controlled trial. Study group(A) received diaphragm strength by power lung device and conventional low back pain exercises in the form of core stability exercises; Curl-Up, Side-Bridge, and Bird-Dog and control group(B) received conventional low back pain exercises for six successive weeks, two times per week. The evaluation tools used were numerical rating scale (NRS), Oswestry disability index (ODI), forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV1/FVC and peak expiratory flow (PEF). Results: There was a significant decrease in NRS and ODI post treatment in both groups compared with that pretreatment (p > 0.001). Also there was a significant increase in FVC, FEV1, FEV1/FVC and PEF post treatment in both groups compared with that pretreatment (p > 0.001). Comparison between groups post treatment revealed non-significant difference in NRS, ODI, FVC, FEV1, FEV1/FVC and PEF (p > 0.05). Conclusion: The application of diaphragm strength combined with the conventional treatment had the same effect as using the conventional treatment alone to manage CMLBP, in terms of pain intensity, functional disability and respiratory function |

| Summary, etc. | الخلفية: آلام أسفل الظهر الميكانيكية يمكن أن تحد من الوظيفة والقدرة لدى الأفراد فيما يتعلق بالحياة والعمل. لذلك ، من المسلم به أنه مشكلة صحية حرجة على مستوى العالم.<br/>الهدف: تهدف هذه الدراسة إلى معرفة تأثير قوة الحجاب الحاجز على مرضى آلام أسفل الظهر الميكانيكية المزمنة. الطريقة: شارك في الدراسة 43 شخصًا تتراوح أعمارهم بين 20 إلى 40 عامًا يعانون من آلام أسفل الظهر الميكانيكية. تم تقسيم الموضوعات إلى مجموعتين. تلقت مجموعة الدراسة(أ) تقوية الحجاب الحاجز وتمارين آلام أسفل الظهر التقليدية في شكل ثلاثة تمارين أساسية للثبات ومجموعة التحكم(ب) تمارين تقليدية لآلام أسفل الظهر. أدوات التقييم المستخدمة كانت مقياس التصنيف العددي ، مؤشر الإعاقة ، السعة الحيوية القسرية، حجم الزفير القسري في ثانية واحدة وذروة تدفق الزفير<br/>النتائج: لم يكن هناك فرق كبير بين المجموعات قبل العلاج (P> 0.05). كشفت المقارنة بين المجموعات بعد العلاج عن اختلاف غير مهم في مؤشر الإعاقة ، السعة الحيوية القسرية، حجم الزفير القسري في ثانية واحدة وذروة تدفق الزفير (P> 0.05). الخلاصة: تطبيق قوة الحجاب الحاجز مع العلاج التقليدي كان له نفس تأثير استخدام العلاج التقليدي وحده لعلاج حالات اسفل الظهر ، من حيث شدة الألم والإعاقة الوظيفية ووظيفة الجهاز التنفسي |
