| Summary, etc. | Background: This is a prospective study to evaluate the role of induction chemotherapy in patients with locally advanced resectable squamous cell carcinoma of the oral tongue (SCCOT) that was conducted at the National Cancer Institute of Egypt. Patients and Methods: From October 2015 to November 2019,sixty-five patients aged 30-71 years with clinical T2-3N0-2M0 SCCOT were planned to receive at least 2 cycles of induction hemotherapy(IC) with docetaxel, cisplatin, and fluorouracil (TPF) protocol followed by radical concurrent chemoradiotherapy or surgery and postoperative radiotherapy based on clinical response to induction (TPF).Fifty-two patients received the planned protocol, the outcome was compared to another historical retrospectively matching group, including 52 adult patients who were offered upfront surgery. Additionally, HPV and Ki67 status were performed for 25 patients in IC group. Results: The clinical response rate to induction chemotherapy was 83.1%. Unfortunately, two patients experienced grade 4 renal toxicity. The clinical response rate in the 52 patients who followed the planned protocol was 94.2%. After a median follow-up of 33 months, there was no significant statistical difference in OS (P=0.620) or DFS (P=0.407) compared to historical upfront surgery controls. Patients who achieved complete clinical response to induction chemotherapy had superior OS and DFS compared to patients who did not. Patients with high Ki67 expression have a significant higher OS and DFS than those with low Ki67 expression. (P=0.009 and 0.034, respectively). Conclusion: Our study showed no improved survival in patients with locally advanced resectable oral tongue SCC stage III or IVA who received induction chemotherapy before definitive treatment in comparison to upfront surgery historical controls. Ki67 can be considered as a biomarker for individualized induction chemotherapy in patients with locally advanced resectable oral tongue SCC. |

| Summary, etc. | يمثل سرطان الرأس والرقبة المركز السادس عالميا من حيث نسبة الحدوث وتمثل أورام تجويف الفم هي النسبة الأكبر ضمن سرطان الرأس والرقبة.<br/>سرطان اللسان هو الأكثر شيوعا في أورام تجويف الفم ونظرا للوظيفة الحيوية له يعتبر علاج أورام اللسان تحديا كبيرا أمام أطباء الأورام وذلك بسبب التأثير السلبي للعلاج على كفاءة حياة المريض وحفظ وظيفة تجويف الفم. ورغم التقدم الملحوظ في أورام اللسان إلا أنه مازالت معدلات الشفاء قليلة على المدي البعيد حيث تصل معدلات الحياة عند خمس سنوات نسبة 50% في أورام اللسان المتقدم. يتم علاج أورام اللسان من خلال لجنة طبية متعددة تضم تخصصات طب الأورام -جراحة الأورام -الأشعة العلاجية-الأشعة التشخيصية. يستخدم العلاج الإشعاعي أو الجراحة في علاج المراحل المبكرة من أورام اللسان بينما في حالات أورام اللسان المتقدم عن طريق الجراحة والعلاج الإشعاعي معا. يمثل العلاج الكيماوي التحفيزي دورا هاما في أورام الرأس والرقبة وذلك للحفاظ على الوظيفة الحيوية وبالفعل تم اعتماده في سرطان الحنجرة. ويمكن للعلاج الكيماوي التحفيزي أن يقلل من حجم أورام الرأس والرقبة ومن ثم نتيجة أفضل جراحيا مما يحسن من جودة حياة المريض ويحفظ وظيفة الجزء المصاب بالورم. |
