| Summary, etc. | Background: Placenta Accreta Spectrum “PAS” is one of the most dangerous conditions of the pregnancy as it is significantly associated with maternal morbidity and mortality. Maternal and neonatal outcomes are generally improved when diagnosis is made before delivery, and the woman is managed by a multidisciplinary team with expertise in the condition. To standardize research terminology and to reduce unanticipated PAS, the European Working Group for Abnormally Invasive Placenta “EW-AIP” developed a consensus checklist (a specialized proforma) for reporting suspected PAS observed during an antenatal ultrasound. The diagnostic accuracy of the European Working Group for Abnormally Invasive Placenta Pro forma has not been assessed.<br/>To make ultrasound (US) documentation more standardized and to establish a systematic approach to the US examination of the placenta.<br/>Objectives: To assess the accuracy of a specialized Pro forma in suggesting the probability of Morbidly Adherent Placenta” MAP”, its extent and its correlation with intraoperative findings.<br/>To evaluate the ability of each sonographic parameter in the prediction of PAS and to correlate this ultrasound assessment to operative judgement of an experienced accreta team.<br/>Materials and Methods: A total of 21 patients diagnosed as having placenta previa with high possibility of placenta accreta. Transabdominal US will be done in addition to transvaginal US to confirm diagnosis of PAS then evaluate the signs of placental invasion. Gray scale and color Doppler will be applied to further evaluate the lower segment vascularity and cervix.<br/>Comparing the ultrasound findings with the operative details through subjective assessment of: invasion (focal or diffuse) by naked eye, vascularity of lower uterine segment and operative time. Documentation of maternal morbidity (bleeding, blood loss by pre-operative and post-operative Hb, blood transfusion, pelvic organ injury and ICU admission) and/or maternal mortality. To assess the clinical significance of our specialized Pro forma and extent of PAS in relation to intraoperative findings.<br/>Results: The overall sensitivity, specificity, Positive Predictive Value” PPV”, Negative Predictive Value” NPV” of our specialized pro forma in assessing extent of PAS (focal, diffuse) in comparison with extent by naked eye was 71.43%, 100.00%, 100.00%, 63.64% respectively.<br/> <br/>The overall sensitivity, specificity, PPV, NPV of our specialized proforma in suggesting the probability of PAS (low, intermediate/ high) in comparison with vascularity of lower uterine segment intraoperatively was 94.12%, 25.00%, 84.21%, 50.00% respectively.<br/>Diagnostic accuracy of US signs in detection of high vascularity of lower uterine segment intraoperative showed that loss of clear zone, abnormal placental lacunae and bridging vessels have the highest accuracy of (80.95%). There was a statistically significant relation between vascularity of lower uterine segment intraoperative and sub-placental hypervascularity by US, with p-value (p<0.05) 0.021.<br/>Conclusion: The 2016 “EW-AIP” specialized Pro forma has a reasonable performance in detecting PAS and assessing intraoperative findings. |
