| Summary, etc. | Background: Diabetic nephropathy (DN) is the chronic loss of kidney <br/>function occurring in those with diabetes mellitus. The estimated glomerular <br/>filtration rate (eGFR) may progressively fall from normal to a point at which the <br/>patient is said to have end-stage renal disease (ESRD). It usually is slowly <br/>progressive over the years. <br/>Individuals with diabetes mellitus have a higher prevalence of hypothyroidism, <br/>and patients with both conditions have a higher prevalence of microvascular <br/>complications. <br/>Aim of the work: This study is conducted for the detection of the effect of <br/>hypothyroidism on the initiation of diabetic nephropathy within five years of <br/>diabetes diagnosis. <br/>Patients and methods: This study was conducted on 80 patients and divided <br/>into two groups; Group (1) 40 middle-aged patients with hypothyroidism and <br/>type 2 diabetes mellitus with 5 years duration of diabetes. Group (2) 40 middle-<br/>aged patients with type 2 diabetes mellitus with 5 years duration of diabetes. <br/>All of them were subjected to thorough history taking, clinical examination and <br/>the following Laboratory tests: Fasting blood glucose, 2 hours postprandial <br/>blood glucose (2h PG), HbA1C, TSH, FT4, ALT, AST, serum creatinine, <br/>cholesterol, and triglycerides, urinary albumin creatinine ratio and fundus examination. <br/>Results: There is no statistically significant difference between the prevalence <br/>of diabetic nephropathy between hypothyroidism and euthyroid diabetic <br/>patients. <br/>Conclusion: In the first five years of diagnosis of diabetes, there is no effect of <br/>hypothyroidism on the prevalence of diabetic nephropathy. |

| Summary, etc. | يعتبر داء السكري من أشهر الامراض في العالم ويؤدي الي مضاعفات كثيرة من اهمها وأشهرها على الاطلاق هو اعتلال الكلي السكري والذي أصبح من اول اسباب الفشل الكلوي المزمن الذي يستلزم الغسيل الكلوي المستمر او زراعة للكلى.<br/>ولذلك فكلما تم اكتشافه مبكرا تمت معالجته عن طريق التحكم في مستوي السكر في الدم وبعض الادوية الأخرى التي تساعد على منع وتأخير حدوثه، ومن اهم وأقدم طرق اكتشاف اعتلال الكلى السكري هو قياس نسبة الزلال ف البول وفحص قاع العين <br/>ومن أشهر امراض الغدد الصماء التي تصيب مريض داء السكري هو قصور الغده الدرقية وكلا منهما يؤثر على الأخر.<br/>وقد اجريت هذه الدراسة للكشف عن تأثير قصور الغدة الدرقية علي بدأ حدوث اعتلال الكلي السكري في المرضي المصريين الذين يعانون من داء السكري خلال خمس سنوات من التشخيص.<br/>و في هذا البحث تم دراسة 80 مريض بداء السكرى من مدة لا تزيد عن خمس سنوات الذين يترددون على العيادة الخارجية بمركز السكر و الغدد الصماء بالقصر العيني منهم 40 مريضا يعاني من قصور بالغدة الدرقية ومعايير الاشتمال للمرضي هي ان يكون مرض السكر من مده لا تزيد عن خمس سنوات منذ تشخيصه وان يكون عمر المريض ما بين 20-60 عاما ومعايير الاستبعاد هي وجود مرض من الأمراض التي تصيب الكلي او الكبد.<br/>وكل هؤلاء الأشخاص تم معرفة تاريخهم المرضي كاملا وتم قياس الوزن والطول ومؤشر كتلة الجسم لهم وأجريت لهم الفحوصات التالية: قياس نسبة السكر بعد صيام 8 ساعات وقياس نسبة السكر بعد تناول<br/> <br/><br/> الطعام بساعتين وقياس نسبة الهيموجلوبين السكري وقياس نسبة الكوليسترول والدهون الثلاثية بالدم وزلال البول وانزيمات الكبد ووظائف الكلى كما انه تم فحص قاع العين لكل منهم.<br/>بناءا على ما سبق فقد انتهت هذه الدراسة الى انه لا يوجد تأثير من قصور الغدة الدرقية على بداية حدوث اعتلال الكلي السكري خلال خمس سنوات من تشخيص داء السكري. |
