| Summary, etc. | Purpose: The aim of the study is to evaluate three-dimensional bone augmentation achieved at maxillary atrophied alveolar ridges using bone marrow aspirate concentrate and mixture of autograft and xenograft versus autograft and xenograft only both using patient specific titanium mesh.<br/>Methods: 5 patients suffered from severe vertical and horizontal full arch maxillary deficiency were subjected to guided bone regeneration. Virtual vertical and horizontal bone augmentation was done for the whole maxillary alveolar deficient ridge using special software to produce virtually augmented stereolithographic plastic models to guide for prebending of titanium meshes preoperatively. In the study group, prebent meshes were loaded with a mix of xenograft and bone marrow aspirate concentrate (BMAC) from the anterior iliac crest, while in the control group the meshes were loaded with xenograft mixed with autograft only with ratio of 1:1. And meshes were fixed in place using mini screws.<br/>Results: Wound healing was uneventful for all patients. CBCT was performed 6 months postoperatively for all patients showing mean vertical bone gain for Study group (3.46±0.89) mm which was higher than the mean value of control group (2.57±0.96) mm Mean value of horizontal bone gain of Study group (3.80±1.20) mm was comparable to the mean value of control group (3.91±0.89) mm. At the re-entry surgery, core biopsies were retrieved from the regenerated sites using trephine burs for histomorphometric analysis. A statistically significant higher bone area percent was detected in the study group (47.9%) as compared to control group (28.5%). A statistically significant higher percentage of mature bone as detected by Masson trichrome was also detected in study group (67.7%) as compared to control group (26.5%).<br/>Conclusion: The clinical results and histomorphometric analysis of the study group were superior to that of the control group concerning the soft tissue healing and the quality of the regenerated bone. Three-dimensional bone augmentation using prebent titanium meshes loaded with xenograft with bone marrow aspirate concentrate could be a reliable less morbid technique. |

| Summary, etc. | الهدف: تقييم زيادة العظم ثلاثية الأبعاد التي تم تحقيقها في الحافة اللثوية للفك العلوي المتضائلة باستخدام تركيز زرع النخاع العظمي ومزيج من الزرع الذاتي والزرع الغريب مقابل الزرع الذاتي والزرع الغريب فقط باستخدام شبكة تيتانيوم مخصصة للمرضى.<br/>المنهجية: تم إجراء تكبير ثلاثي الأبعاد للعظام في الفك العلوي باستخدام شبكات تيتانيوم مخصصة لكل مريض على 5 <br/>مرضى (3 ذكور و2 إناث) بمتوسط عمر 38.2±2.39عام تم تخصيص المرضى لإجراء تكبير كامل لقوس الفك العلوي في الأبعاد الرأسية والأفقية بدراسة تقسيم الفم، حيث يعتبر الجانب الأيسر الجانب الدراسي والجانب الأيمن الجانب الضابط لجميع المرضى. • مجموعة الدراسة (الجانب): تكبير العظام الرأسي والأفقي باستخدام مركز طحن نقي العظام (BMAC) وزرعة عظمية بقرية جزئية باستخدام شبكة تيتانيوم مخصصة للمريض. • مجموعة الضابط (الجانب): تكبير العظام الرأسي والأفقي باستخدام مزيج من زرعة جزئية ذاتية المنشأ وزرعة عظمية بقرية جزئية فقط باستخدام شبكة تيتانيوم مخصصة للمريض.<br/>النتائج: بعد مرور 6 أشهر، تم اكتشاف نسبة مساحة العظم الأعلى بشكل ذو دلالة إحصائية في مجموعة الدراسة كان 47.93±3.48% مقارنةً بمجموعة المقارنة كانت 28.53±5.92%. تم أيضًا اكتشاف نسبة أعلى بشكل ذو دلالة إحصائية من العظم الناضج وفقًا لتحليل Masson trichrome في مجموعة الدراسة 67.73±16.58 % مقارنةً بمجموعة المقارنة كانت 26.56±6.22 %. |
