| Summary, etc. | Accurate staging of cancers are very crucial, as it determine the <br/>appropriate treatment lines to achieve the best results and avoid <br/>preventable morbidity and mortality related to unnecessary laparotomies. <br/>Pre-operative radiological assessment helps in identification of many of <br/>advanced cases and diagnostic staging laparoscopy can catch the missed <br/>lesions. <br/>Florescence imaging guided surgery (FIGS) is a technique which may <br/>help and add value over standard diagnostic laparoscopy. <br/>Indocyanine green is a safe option act as a color code for hyper-vascular <br/>lesions by using near infra-red (NIR) specialized camera system. <br/>In this study, 30 patients with operable intra-abdominal malignancy had <br/>been injected with 0.25-0.5 mg/kg 24-72 hours before surgery. <br/>Results: <br/>12 lesions had been detected in 5 patients; 8 lesions were diagnosed pre-<br/>operative, while 11 lesions were detected intra-operative by using FIGS <br/>and ICG-NIR (sensitivity 92%).7 lesions had been sent for <br/>histopathological assessment, 4 were metastatic with specificity (57%). <br/>2 patients had been discovered intra-operative to have peritoneal nodules, <br/>one of them detected by ICG. These results affect surgical decision in 2 <br/>patients. |

| Summary, etc. | مرضى أورام الجهاز الهضمى تتشابه أعراضهم مع الكثير من أمراض الجهاز الهضمي المعتادة مما قد يؤدي إلي تأخر التشخيص وتقدم المرض أو انتشاره, ومن هنا تأتي أهمية الأشعه والفحوصات لتحديد الدرجة التشخيصية للورم وتحديد الخطة العلاجية الأمثل.<br/>٩٠% من حالات الوفاه من مرضى الأورام الخبيثه تكون مصاحبة لوجود ثانويات للورم . طبقاً لنظرية انتشار البذرة والتربه الخصبه ، يمثل الكبد والغشاء البريتوني النسبة الأعلى من حيث أحتمالية لوجود ثانويات. <br/>يعتبر الفحص التشخيصي بالمنظار الجراحي حل أمثل و آمن للتأكد من التشخيص ودرجة تقدم الورم و إستبعاد وجود ثانويات تجنباّ لإجراء جراحات متقدمة ذات خظورة عالية دون جدوى تذكر . ولكن يصعب في أوقات عدة إجراء فحص شامل بدقة لعدة عوامل منها. السمنة المفرطة أو المناطق العميقة صعبة الوصول الحوض ولذا لزم الأمر أن يتم إيجاد وسائل مساعدة تزيد من دقة الإستكشاف وتقلل من إحتمالية وجود ثانويات غير ملوحظة.<br/>تعمل الصبغات المضيئة كشفرة لونيية مضيئة لرسم وتحديد أماكن الأورام، العقد الليمفاويه الحارسه، والثانويات. <br/>تتميز صبغة الإندوثيانين الأخضر بالأمان وندرة إحتمالية حدوث مضاعفات. تعمل عن طريق السريان بالدم والإرتباط بالروتينات، يتم إستخلاصاها عن طريق الخلايا الكبدية وتفرز مع العصارة الصفراوية وتصدر موجات ضوئية يتم إلتقاطها _بإستخدام كاميرا مخصصة للأشعه تحت الحمراء بتردد لا يمكن للعين المجردة في إلتقاطه تحت تأثير الأشعه الضوئية المعتادة. في هذا البحث تم حقن ٣٠ مريض _بعد شرح كافة التفاصيل وأخذ موافقة مكتوبة_ بالصبغة وريديا (٢٥.٠ – ٥.٠ مجم/كجم) ٢٤-٧٢ ساعة قبل الجراحة ومن ثم يتم إستخدام التقنية في البحث عن وجود ثانويات بالكبد أو الغشاء البريتوني في مرضى الأورام الباطنية. |
