| Summary, etc. | Genetic studies of hematological neoplasms are becoming more significant <br/>and gaining so much interest regarding diagnosis, prognosis and target therapy. In <br/>our study, ACE2 gene expression was higher in cases of AML positive for COVID<br/>19 infection than AML negative for COVID 19infection. So, in our study, we studied <br/>the expression of ACE2 in AML Egyptian patients and half of our patients were <br/>COVID 19 positive and the other half was COVID 19 negative. The study included <br/>78 patients, median age was 44 years (18-72 years)12 females, and 66 males AML <br/>patients were recruited to the study, to help us perceive a broader image of myeloid <br/>leukemias and the effect of ACE2 genes expression on prognosis of AML patients <br/>together with other prognostic markers in cases of COVID 19 positive patients<br/>versus COVID 19 negative patients.<br/>The aim of our work to measure ACE-2 gene expression by real time PCR <br/>among De novo acute myeloid leukemia patients with and without COVID-19 <br/>infection to study the disease severity and prognosis.<br/>Relation of ACE2 gene to initial laboratory prognostic parameters as blood count, <br/>BMA findings, FAB classification, karyotyping, IPT, and clinical data (fever, <br/>organomegaly, Diabetes and hypertension) were evaluated. <br/>In this study there was male predominance (77% males and 33% females).<br/>In this study, Patients >=45 years old were worst survival than patients <45 <br/>years old 3.6 months versus 10 months (p0.006).<br/>Regarding FAB classification. In this work AML M2 was the most common <br/>subtype 33/78 (42.3%) followed by M4 23/78 subtype (29.5%).<br/>Summary and Conclusion <br/> _____________________________________________________________________<br/>143<br/>In our study, ACE2 gene expression was higher in cases of AML positive for <br/>COVID 19 infection than AML negative for COVID 19 infection. 37/39 (97.4%) of <br/>COVID 19 positive patients were high expressors of ACE 2 gene and only 1/39 <br/>(2.6%) of COVID 19 negative patients was high expressor of ACE2 gene P value <br/>(<0.001).<br/>In our study, Patients entered in CR were better survival than patients who did <br/>not enter in CR 15.9 months versus 2 months (p<0.001).<br/>We found that in COVID-19 positive cases there was positive correlation <br/>between ACE 2-fold change and DFS as high expressors of ACE 2 gene patients had <br/>better DFS than low expressors of ACE2 gene 6.9 versus 0.9 P (0.025).<br/> |

| Summary, etc. | سرطان الدم الميلودي الحاد هو أحد أورام الدم الأكثر شيوعًا، خاصة بين البالغين. تلعب التعديلات الجينية دورًا رئيسيًا في التسبب في الإصابة بسرطان الدم الميلودي الحاد ومسار المرض والاستجابة للعلاج.<br/>يعمل ACE 2 كمنظم سلبي لمكونات RAS تم قياس التعبير الجيني لمكونات RAS بواسطة PCR في عينات النخاع العظمي ووجد ان التعبير الجينيل ACE 1 , ACE2 في سرطان الدم الميلودي اعلي من سرطان الدم الليمفاوي. تم اكتشاف دور أخر للأنزيم ACE 2 في عام 2002-2003 بواسطة وباء سارس كورونا فيروس لان ACE 2 يعمل كمستقبل لدخول الفيروس داخل الجسم.<br/>الهدف من الدراسة: دراسة التعبير الجيني لـ ACE 2في مرض ىسرطان الدم الميلودي الحاد لدى البالغين وارتباطهم بالبيانات السريرية للمرضى والاستجابة للعلاج في حالات الإصابة بفيروس كورونا وفي حالات عدم الإصابة بفيروس كورونا.<br/>الموضوعات والطرق: تم جمع العينات من عيادة الأورام الطبية بالمعهد القومي للأورام. تم إجراء الدراسة وتحليل البيانات في قسم علم الأمراض الإكلينيكي بالمعهد القومي للأورام. أجريت الدراسة على عدد39 مريضًا من مرض ىسرطان الدم الميلودي الحاد ممن أصيبوا بفيروس كورونا وعدد 39 مريضا لم يتم اصابتهم بفيروس كورونا.<br/>تم الحصول على عينات نضح النخاع العظمي من مرض ىسرطان الدم الميلودي الحاد لاستخراج الحمض النووي الريبي RNA وتحويل الي الحمض النوويDNA للتحضير لتقدير تعبير ACE 2.<br/>تم فحص مرض ىسرطان الدم الميلودي الحاد من أجل التعبير الجيني ACE 2 باستخدامPCRفي الوقت الفعلي وتم مقارنه نتائج المرضى نسبه الي نتائج الحالات الضابطة للحصول على التقدير الكمي النسبي(RQ).<br/>النتيجة: المرضي كبار السن أكبر من او يساوي 45 سنة كانوا أسواء في البقاء على قيد الحياةمن المرضي الأصغر من 45 سنة 3.553 شهرا ضد 10 شهور P= (0.006) في هذه الدراسة وجد ان التعبير الجيني ACE 2 أعلي في حالات سرطان الدم الميلودي وقد أصيبوا أيضا بفيروس كورونا من حالات سرطان الدم الميلودي ولم يصابوا بفيروس كورونا حيث ان 97.4% من مرضي فيروس كورونا لديهم تعبير جيني عالي P (<0.001).<br/>لقد وجدت علاقة إحصائية بين حياة المريض (البقاء على قيد الحياة) ووصول المريض الي انخفاض نسبة الخلايا السرطانية الي اقل من 5% في اليوم الثامن والعشرون من بداية العلاج (P<0.001).<br/>لقد وجدت علاقة إحصائية بين تعبير ACE 2 والبقاء على قيد الحياة في الحالات الإيجابية لفيروس كورونا (P=0.025).<br/>ووجد ان أفضل المرضي من حيث البقاء على قيد الحياة هم اللذين يمتلكون إفراط في التعبير ACE 2 ونقل (8و21) P (0.028). <br/> |
