| Summary, etc. | This randomized controlled trial study was conducted on 34 patients with medial malleolar fractures either isolated as a unimalleolar fracture or part of a bimalleolar or trimalleolar fracture who were split into two groups, the first was fixed with 4.0mm cannulated cancellous fully threaded screws in positional mode and a second group that was fixed with 4.0mm cannulated cancellous partially threaded screws in lag mode.<br/>The youngest patient included in our study was 18 years old while the oldest was 71 years old. The mean of age was 36.9 ±15. The shortest follow- up period was 6 months and the longest was 18 months. The time interval between trauma and time of surgery was done was within the first day of trauma and the maximum days was 7 days. Our study showed that the mean time for the patient to fully bear weight and walk without pain was 28.1 ± 8.6 days in fully threaded screws group and 31.8 ± 9.8 days in partially threaded screws group.<br/>The mean time of fracture union was 6.23±1.3 weeks for the fully threaded group and 6.47±1.5 weeks for the partially threaded group. 5 patients(14%) in total had complications in the form of superficial infection in our study that recovered after prescribing antibiotics with repeated dressings. 2 were from the partially threaded group and 3 were from the fully threaded group. A statistical significant difference (p-value = 0.024) was found between studied groups (fully and partial threaded screws) as regard 4 weeks AOFAS. It was 67.6± 8.5 in fully threaded screws group and 61.5 ± 7.1 in partially threaded screws group. This indicates that patients treated with the fully-threaded screws achieved a higher functional score in the form of AOFAS score after 4 weeks of treatment. The score later at 3 months and 6 months follow-up showed no significant difference.<br/>No secondary procedures were needed for any of the patients. |

| Summary, etc. | تعد كسور الكاحل واحدة من أكثر الإصابات شیو ًعا التي یواجھھا جراح العظام أثناء ممارستة. یمكن إصلاح كسور الكاحل الداخلیة إما بمسامیر ملولبة جزئیا أو بمسامیر ملولبة بالكامل. ھدف الدراسة ھو مقارنة الوسیلتین لاكتشاف افضل طریقة لاصلاح الكسور الداخلیة للكاحل. تمت متابعتھم وإصلاحھم بإحدى الطرق شملت ھذه الدراسة 34 جزئیًا مریضاً المذكورة سابقا . تم متابعة كل الحالات لمدة لا ،ً بعد تقسیمھم بالتساوي إلى مجموعتین تقل عن 6 اشھر. في اثناء المتابعة یتم النظر الى الاشعات و تسجیل التئام الكسر, و كذلك متابعة استعادة المریض للقدرة الوظیفیة, و ذلك بتسجیل نتیجة نقاط الجمعیة الأمریكیة لجراحة عظام القدم والكاحل (score AOFAS(. أثبتت ھذه الدراسة أن كلتا الطریقتین كانتا فعالتین في علاج الكسور. ومع ذلك، ھناك أدلة على أن المسامیر الملولبة بالكامل تساعد في تحقیق نتائج وظیفیة أفضل في الشھر الأول بعد العملیة الجراحیة, حیث ان مجموع نقاط مجموعة المسامیر الملولبة بالكامل ±67.6 ,8.5 بینما كان مجموع نقاط مجموعة المسامیر الملولبة جزئیًا 61.5 ± .7.1 و لكن لم یكن ھناك فرق كبیر عندما تم قیاس النتیجة في 3 و 6 أشھر. أظھرت كلا المجموعتین عدم وجود فرق كبیر في المضاعفات ولم تضطر أي من الحالات المشمولة في دراستنا إلى الخضوع لعملیة جراحیة مرة أخرى لإزالة المسامیر. . ﻓﻲ اﻟﻨﮭﺎﯾﺔ، ﻗﺪ ﺗﺴﺎﻋﺪ ال اﻟﻤﻠﻮﻟﺒﺔ ﺑﺎﻟﻜﺎﻣﻞ ﻋﻠﻰ ﺷﻔﺎء ﻛﺴﻮر اﻟﻜﺎﺣﻞ ﺑﺸﻜﻞ أﺳﺮع. <br/>في اثناء المتابعة یتم النظر الى الاشعات و تسجیل التئام الكسر, و كذلك متابعة استعادة المریض للقدرة الوظیفیة, و ذلك بتسجیل نتیجة نقاط الجمعیة الأمریكیة لجراحة عظام القدم والكاحل (score AOFAS(. أثبتت ھذه الدراسة أن كلتا الطریقتین كانتا فعالتین في علاج الكسور. ومع ذلك، ھناك أدلة على أن المسامیر الملولبة بالكامل تساعد في تحقیق نتائج وظیفیة أفضل في الشھر الأول بعد العملیة الجراحیة, حیث ان مجموع نقاط مجموعة المسامیر الملولبة بالكامل ±67.6 ,8.5 بینما كان مجموع نقاط مجموعة المسامیر الملولبة جزئیًا 61.5 ± .7.1 و لكن لم یكن ھناك فرق كبیر عندما تم قیاس النتیجة في 3 و 6 أشھر. أظھرت كلا المجموعتین عدم وجود فرق كبیر في المضاعفات ولم تضطر أي من الحالات المشمولة في دراستنا إلى الخضوع لعملیة جراحیة مرة أخرى لإزالة المسامیر. |
