| Summary, etc. | Background: Trismus is a common oral morbidity that accompanies oncological treatment in head and neck cancer patients (HNCs), and it is defined as inability to fully open the mouth that is why it is known to deteriorate the general health significantly while negatively affecting the quality of life (QOL), it is also reported as the second most burdensome morbidity for HNCs. Studies have reported that laser therapy was able to manage oral complications effectively in cancer patients. Purpose: The Objective of this study was to identify the effect of high-power laser therapy (HPLT) on Radiotherapy induced Trismus (RTIT) in HNCs. Design: open label pre– test post –test randomized controlled trial. Methods: A total of forty-four patients, diagnosed with RTIT were randomly allocated in to 2 groups with 22 patients in each. Study group (A): received HPLT in combination with conventional physical therapy programme. Control group (B): received conventional physical therapy programme only. Outcome measures: Visual Analogue scale (VAS) was used to measure pain level, the University of Washington Quality of life questionnaire (UW–QOL) was used to measure QOL, and the vernier calliper scale (VCS) was used to measure the amount of mouth mandibular opening (MMO) before and after physical therapy treatment sessions.<br/>Results: statistical analysis showed that a significant difference of all variables was found in patients in the study group treated with HPLT along with conventional physical therapy programme than the control group who treated with conventional exercises only, the UWQOL showed significant difference between the two groups in favour to the study group with (p<0.05), the VAS showed a significant reduction from 8.77 ± 0.81 to 2.31 ± 1.04 with (p<0.05) and a degree of enhancement of 73.66% compared to 16.22 in favour to the study group, the MMO showed significant difference for the study group with (p<0.05) and 85.56%enhancement compared to 37.11% for the control group. Conclusion: HPLT in combination with conventional physical therapy programme are more effective in treatment of RTIT than conventional exercises alone. |

| Summary, etc. | المستخلص<br/>الخلفية: تيبس الفك الناتج عن العلاج الاشعاعي لأورام الرأس والرقبة, هو عبارة عن مرض فموي شائع يصاحب علاج الأورام لدى مرضى سرطان الرأس والرقبة، ويتم تعريفه على أنه عدم القدرة على فتح الفم بالكامل ولهذا السبب من المعروف أنه يؤدي إلى تدهور الصحة العامة بشكل كبير مع التأثير سلبًا على جودة الحياة. تم الإبلاغ عنه أيضًا باعتباره ثاني أكثر الأمراض إرهاقًا بالنسبة لمرضى أو رام الرأس والرقبة. أفادت الدراسات أن العلاج بالليزر كان قادرًا على إدارة المضاعفات الفموية بشكل فعال لدى مرضى السرطان.الهدف من البحث: دراسة تأثير جهاز الليزر عالي الشدة على الألم وتيبس الفك وجودة الحياة الناتج عن العلاج الاشعاعي لمرضى أو رام الراس والرقبة.تصميم البحث: دراسة سريريه محكومه عشوائيه. المشاركون والعلاج: شارك في البحث عدد44مريضا يعانون من تيبس الفك الناتج عن العلاج الاشعاعي لأورام الراس والرقبة، 22 في مجموعة الدراسة أو مجموعة التطبيق و22 في المجموعة الضابطة وتم توزيعهم بطريقه عشوائية غير انتقائية ,اعمارهم ما بين(20ال 65) عاما من كلا الجنسين سيدات ورجال حيث ان المجموعة العلاجية تلقت جهاز الليزر عالي الشدة بالإضافة إلى تمارين الفك التقليدية والمجموعة الاخرى (الضابطة) تلقت تمارين الفك التقليدية فقط وتم عمل تسجيل ومتابعه للمجموعتين عن طريق قياس فتحة الفم وشدة الألم وملئ استبيان جودة الحياة قبل اجراء العلاج وبعد تلقي العلاج. الطريقة: عن طريق قياس شدة الالم بمقياس التحليل البصري لشدة الألم وقياس مقدار فتحة الفم عن طريق القدمة ذات الورنية وقياس تحسن جودة الحياة باستخدام استبيان قياس جودة الحياة قبل بدء العلاج وبعد انتهاء العلاج المكون من الليزر عالي الشدة بمقدار4.29جول\سم2,وطول موجي (808 و905 نانومتر) عن طريق جهاز ليزر من شركة ASA LASER ولمدة 2.39 دقائق على مفصل الفك من الجانبين الايمن والايسر للمريض وتردد 700 هرتز واجمالي الطاقة 3وات ,ويكون المريض في وضع الجلوس بشكل مستو بالإضافة إلى التمارين التقليدية لمفصل الفك للمجموعة الأولى والمجموعة الثانية تلقت التمارين التقليدية لمفصل الفك فقط.الاستنتاج: في هذه الدراسة كلا المجموعتين أظهرت تحسنا ملحوظا على مستوى زيادة مقياس فتحة الفم ومستوى الألم وتحسن استبيان قياس جودة الحياة ولكن استخدام الليزر عالي الشدة أظهر تحسنا بارزا على مستوى الألم وزيادة مقدار فتحة الفم بالمقارنة بأداء التمارين العلاجية التقليدية لمفصل الفك فقط. |
