| Summary, etc. | After resection of tumors in mandible, they require reconstruction in order to restore function <br/>and aesthetics. The most reliable reconstructive option for resected mandible is the free fibula <br/>flap (FFF) due to its compact bone structure, its length that enables reconstruction of large <br/>defects and the possibility of future implant placement in order to restore the function by placing <br/>dental prosthesis (Fixed or removable).<br/>The main disadvantage of free fibula flap is the limited vertical bone height it provides compared <br/>to dentate mandible. Many techniques have addressed this problem such as double barreling, <br/>vertical distraction osteogenesis and onlay grafting of fibula with iliac crest.<br/>This study aimed to provide a novel solution to such problem using inlay grafting of the FFF by <br/>using a non vascularized fibula graft from the same side to be sandwiched between osteomatized <br/>fibula flap.<br/>This study included 7 patients with resected mandible due to benign tumors and indicated for <br/>mandibular reconstruction using FFF<br/>The study was performed at the department of plastic and reconstructive surgery, faculty of <br/>medicine, Cairo university and the patients were recruited from the department of oral and <br/>maxillofacial surgery, Faculty of Dentistry, Cairo university<br/>The FFF was harvested, split and sandwiched by prefabricated surgical guides and fixed to the <br/>mandibular stump by 2.7mm reconstruction plate <br/>The results showed statistically significant increase in the vertical bone height of augmented <br/>segment of fibula compared to non augmented fibular segment and patient satisfaction.<br/><br/><br/> |

| Summary, etc. | أ. الهدف: الهدف الرئيسي من اجراء هذا البحث هو تقييم التدعيمالعظمي الطولي للفك السفلي باستخدام سديلة شظية موعاه مدموجه مع غير موعاه، بالاضافة الى قياس مدى رضا المتطوعين عن النتائج.<br/>ب.المنهجية: 7 حالات تم استئصال جزء من الفك السفلي لهم بسبب اورام حميدة و يتعين اجراء تعويض للجزء المستأصل باستخدام سدائل شظوية حرة موعاة تم تصميم دليل جراحي و طباعته لتصميم السديلة و تطويعها لشكل الفك السفلي و زيادة طول الجزء الامامي من السديلة لاستيعاب زرعات الاسنان و اجراء العملية الجراحية تحت التخدير الكلي, و تم اخذ اشعة مقطعية للوجه بعد الاجراء الجراحي مباشرة و بعد 6 شهور و بعد عام كامل لقياس تغير طول الجزء الامامي من السديلة المضاف مع تسجيل رضاء المرضي بعد الاجراء الجراحي باسبوع و بعد شهر, و 6 شهور , و عام كامل <br/>جـ . النتائج : متوسط مقدار الزيادة في طول الجزء الامامي من السديلة الشظوية الحرة مقارنة بالجزء الغير مدعم من السديلة هو <br/>7.54مم±1.13مم<br/>متوسط مقدار النقصان في طول الجزء الامامي من السديلة بعد عام<br/>1.61مم±0.34مم<br/>متوسط التحسن في رضاء المريض بعد عام<br/>9.57±0.53<br/>د. الاستنتاجات:هذه التقنية نجحت في زيادة ارتفاع الجزء المدعم من السديلة الشظوية مما يسمح مستقبلا بغرس زرعات اسنان للجزء المدعم منها بصورة افضل مما يترتب عليه تعويض الاسنان المفقودة مع الجزء المستأصل من الفك السفلي و استعادة الوظيفة و الشكل الجمالي للفك السفلي.<br/><br/> |
