| Summary, etc. | Background: The optimal treatment approach for Legg-Calvé-Perthes disease (LCPD) remains a subject of debate. This meta-analysis aims to determine whether Shelf acetabuloplasty yields superior radiographic and clinical outcomes compared to alternative treatments for LCPD. Aim of the Work: The primary objective is to assess the efficacy of Shelf acetabuloplasty in improving radiographic parameters and functional outcomes in patients with LCPD. Patients and Methods: Twelve studies were included in this meta-analysis, comprising both qualitative and quantitative analyses. A total of 450 hips, with an average age of 7.16 years (range: 2 to 18.4 years), were evaluated. The mean follow-up duration was 7.65 years. Various pre-operative classifications, including Catterall and Herring classifications, were utilized. Post-operative outcomes such as the Stulberg classification, Iowa hip score, Central edge angle, Sharp angle, Reimers migration index, and acetabular coverage were assessed across multiple studies. Results: Shelf acetabuloplasty demonstrated favorable outcomes in the majority of patients, with significant improvements noted in radiographic parameters and functional scores. Specifically, the Stulberg classification revealed that 48% of patients achieved spherical hips (Stulberg I or II), indicating a good prognosis. Furthermore, the Iowa hip score significantly improved post-operatively. Meta-analyses of parameters such as the Central edge angle, Sharp angle, and acetabular coverage indicated positive trends, although some comparisons did not reach statistical significance. Conclusion: Our meta-analysis supports Shelf acetabuloplasty as an effective surgical intervention for LCPD, leading to improved radiographic outcomes and functional scores. However, further clinical trials are warranted to strengthen the evidence base and facilitate comprehensive systematic reviews and meta-analyses in this field. |
