| Summary, etc. | Background: Osteochondral injuries commonly affect the ankle joint and involve the dome of the talus1. The purpose of this study was to assess the outcome of autologous transplantation of cancellous tibial graft with periosteal transfer in large OLTs<br/>Methods: Thirty patients (17 females, 13 males), (range, 18-50 years), with a large and recurrent OLT (>150 mm2 ) were treated with autologous bone graft and periosteum transfer. The procedure consisted of malleolar osteotomy, curettage of sclerotic bone, autologous bone graft and transfixion of periosteum taken from distal tibia over the graft. Outcome measures, includ the pain visual analog scale (VAS), American Orthopaedic Foot & Ankle Society (AOFAS) score, and Foot & Ankle Disability Index (FADI).<br/>Results: There were significant improvements in VAS pain score from 7.07(+1.47) before surgery to 0.43(+0.68)in the last follow up after surgery and . The AOFAS and FADI scores were also significantly improved from 40.6(+11.3) and 53.77(+5.97) preoperatively to 95.13(+3.43) and 93.8(2.95) postoperatively at the last follow up, respectively with a follow up period of (6:18 Month). <br/>Postoperative complications: included one patient who required removal of medial malleolar osteotomy screw due to painful implant. There were no nonunions or malunions of the osteotomies and no donor site complications .<br/> Conclusion: Autologous bone graft and periosteum transfer was an effective treatment for large OLTs leading to significant decreases in pain and improvement in functional scores .<br/> Level of Evidence: Level IV, prospective case series. |

| Summary, etc. | تم اجراء دراسه اكلينكيه عشوائيه مستقبليه لمعرفه نتائج علاج قرح عظمه القنزعي الكبيره والمتكرره بواسطه رقعه عظميه ذاتيه ورقعه سمحاقيه <br/>شملت هذه الدراسه 30 مريضا وقد تم اختيار جميع المرضي وفق المعايير الاتيه :-<br/>1.السن من 18 الي 50 سنه.<br/>2.ان يكون المريض قد خضع لعلاج تحفظي 6 اشهر علي الاقل.<br/>تم استبعاد المرضي الذين تحققت فيهم المعايير الاتيه :-<br/>1.حدوث التهاب صديدي بالمفصل.<br/>2.مرضي الامراض الروماتيزميه مثل الروماتويد المفصلي.<br/>3.مرضي الاعتلال العصبي بالقدم والكاحل. <br/>4.المرضي الذين يعانون من التشوهات الزاويه بالكاحل.<br/>حيث تم استخدام جرح مقوس علي الحد الخلفي الانسي لاقصي الظنبوب كما استخدم كسر الكعب الانسي للوصول الي مكان الاصابه كما اخذت الرقعه من اسفل عظمه القصبه واخذت منه ايضا الرقعه السمحاقيه.<br/>خضعت الدراسه لبروتوكول اعاده التاهيل والمتابعه وكان متوسط فتره المتابعه من (6-18)شهراً.<br/>وقد تم عمل مقياس جمعيه جراحه القدم والكاحل الامريكيه للمرضي (AOFAS) وايضا مؤشر العجز لانشطه الحياه اليوميه للقدم والكاحل (ADL,FDI).<br/>وقد استنتجنا أن علاج علاج القرح الكبيره والمتكررة لعظمه القنزعي بالكاحل بواسطه رقعه عظميه ذاتيه ورقعه سمحاقيه مفيد وفعال. |
